Your BM starts off great, thick, hard smooth, large stool and if I could stop there it would be great, but then I feel I have to go some more and it gets much softer and mushier followed by spasms. I seem to get this about 3 out of 7 days per week.

haven't had it change on me in the middle, but it often changes from on bathroom trip to the next. Least for me... Just figured it was the difference it what i ate. Never really thought about it that much.

This happened to me last night (sorry TMI). I was really bloated before I got into bed and about 30 minutes later, you guessed it, I was on the toilet. It wasn't so bad because it was still early and afterwards the bloating and gas were gone. I don't know what can be done about it... I used a canasa after the D and this morning I had loose BM but no blood or mucus. I think the iron I've been taking has made my BMs looser.
Diagnosed with proctitis in March 2007 - Treated with Canasa 2x Daily
Diagnosed with UC December but had symptoms months ago - Treating with Asacol (feel hopeless)

I have this happen to me in the morning, but not just during one BM. The BMs get looser as the morning goes on.

If I had spasms (in the rectum are you meaning?)...I'd be obviously flaring.

Are you on the rectal meds nightly?

I was on cortifoam nightly fro a 2 week period which got the bleeding under control and greatly improved my BM...but I was instructed to go to every other night so I skipped last night and had the symptoms above. Maybe I should stay on it nightly for another week? The spasms I get are in my abdomen but they only come after I have a BM and never before; perhaps the reasom I have to keep going when i should really have stopped is due to rectal spasms??? Peppermint capsules really help settle the spasms down thank God and it's great not to have to take more rx's to do so.

Oh, I assumed rectal spasming. It could just be IBS which causes spasming (like peristalsi gone wild). I think you should stay at the every second night and stay there a few times after you lower the dosage of pred).

One thing about peppermint....it will slow down digestion...maybe ease up on them and consider a digestive enzyme or maybe fennel seeds or something like that.

I'd chalk it up to it being the nature of the beast...I've had times where sitting on the toilet both formed stools came out with rushing D together and that by far has been the weirdest feeling/exepriance I've had when it comes to stools.

My bum is broken....there's a big crack down the middle of it! LOL :)

Q, I am not on pred and I do take digestive enzymes but last night I skipped it... Even so I get the spasms with or without.

Cfromutah, I do feel I am getting closer to acheiving remission but I am under tremendous amounts of stress...I am doing my best to try and control it but the things I must deal with each day can be overwhelming even for the healthiest people. I was actually in remission in Oct when I had my last scope but the stress hit hard and my health suffered hence my flair. I look forward to either trying something like remmicaide or having surgery as I cannot keep living like this.

Thanks to all for your support and thoughts...I pray each day for everyone and anyone/thing that are suffering.

Remicade has worked pretty well. I'm still having morning urgency issues, but my GI thinks it's IBS. I've been keeping a food diary and haven't found any connections yet, so I don't know. I'm also having random nausea and some hair loss, but I don't know if that's the Remicade, Colazal, or just this darn disease in general.

Glad to hear it's working...I think I have Ibs too...isn't enough to deal with UC LOL...I think ibs is just a more severe form of ibs and I've been saying so for years...I wonder what the % of those than have bad ibs get ibd?

There is a gray area between remission and flare. It sounds as if you have approached it or are already there. There probably is still some inflammation that is still impeding the water absorbtion process leading to the loose part of the BM.

Whatever you do don't strain if you feel some tenesmus coming on. Even if it requires a little more time in the bathroom or some deep breathing exercises to calm the spasms or urge.

Keep doing what seems to be working and my best wishes for your "break" into remission.

thanks uc warrior...what you say makes much sense...I will try to stop the urge to pass the additional stool as that's when the trouble starts, it's just very difficult to control especially once I start going. Maybe I should keep using the cortifoam daily vs every other night for a few more days. I don't know of I have tenesmus as I pass a large amount of stool after passing a large amount to begin with.

I am not sure I know what a spasm is. I do have the formed stool, then the loose stool in one movement but I don't think I have spasms.
 
What's a spasm?
 
I agree about the relaxation and breathing thing. Whenever I am feeling bad if I close my eyes and breathe deep, usually things work themselves out quickly

When I'm in the gray area and have the ability to use the bathroom as much as needed I have had good success by relieving the immediate BM without straining to get anything else out. Do the deed and get off toilet. I go on about other business and return if the urge returns. This increases trips but makes each trip more successful. At this stage the desire/struggle to move more stool, whether productive or not, is not the best course of action. This is where you have to actively fight this disease on its own turf.

I've noticed that the more I strain to get rid of the additional stool makes the flare worse. Sometimes I used to get cramps which made me think I still had to pass something. I've learned not to do that.

     That happens to me when beginning a flare.  I would start back on the rectal meds every night for a while.  bbc..do you mean to say you think IBD is a more severe form of IBS?  It certainly is.

Update 11/03/08...finally in remission!!  Hope I don't jinx myself.  Off the prednisone since 11/01/08...now see if I can stay off for longer than two weeks.  Other meds: 6MP (75 mgm), colazal (6 per day), Benicar and Toprol (high blood pressure meds), Probiotic (2 per day), fish oil capsule and multi-vitamin and calcium with vitamin D.

Yes, I meant to say IBD is a severe form of IBS...

I will go back to everynight on the cortifoam for a few more days before going to every other night.

I do not have to strain at all, once the first part (formed hard etc) has past I feel as if I have to go more and it comes out without much provacation just mushy and as uc warrior suggests it has not had enough time to get all the liquids, nutrients etc out...hence the soft stool and weight issues.

A spasm is not a sharp pain like a cramp but contractions going on in your colon...they hurt and are very draining to one's energy levels....peppermint really helps quite a bit.

Technically IBS and IBD are 2 different entities with some similar symptoms but to date as far as researchers are concerned they are not in the same catagory what so ever...but it is possible for one patient to have both an IBD and IBS.

that has happened to me before, i would do canasa after that just to keep things calm.

Thanks, I 've some canasa left...maybe I'll alternate beween those and the cortifoam as my gi told me to try it with rowasa at one point even though the rowasa did not work as well as the cortifoam.

Just wanted to add that this happens to me, too.  I think everything is going great, and then all of a sudden there's D.  Very frustrating when you think you're finally having a normal bathroom experience.

DX:  Pancolitis as of 9/08 (Proctitis as of 1/08)
Current Treament:  Spinach/sunflower & Low-residue diet.
Previous Treatment:  Canasa suppositories, Cortifoam, Prednisone, Colazal, Symex DuoTabs, Rowasa, Bentyl, Cortenema, Asacol, Iron, Zoloft, Acidophilus.

after over a year...... what is normal???? I know what normal was before... But what is normal now?
