I think I am starting to reach the end of my rope here. I know there are so many other people here that have dealt with this for much longer, but I am starting to get seriously frustrated. Almost a year of my life has been put on hold because of the constant flare and my worries of accidents etc. I thought I was starting to do well a few weeks ago and then all of a sudden these intense and painful muscle spasms started when I had a BM. Most of the time nothing wouold come out. It was almost like dry-heaving out of your ass. I don't complain to much but I am starting to get a little frustrated. Anyone experience this? BTW, I am going to the bathroom at like 20x a day again. I was down to 8 but I am thinking my reduction in mg's of Pred might have something to do with it. I never had good results going from 20mg to 15mg. Thanks everyone and have a gerat holiday!

What you describe is not unusual. If you're back up to 20x a day, I'd talk to your doctor about bumping back up on pred for the time being. What other meds are you on?
 
Have you tried any supportive therapies like omega3 supplementation to reduce inflammation or probiotics? What's your diet like?
 
In my experience, the key to reducing or getting off meds is addressing all the other factors: removing or reducing stress in your life, reducing inflammation, healing the gut, rebalancing healthy gut ecology and removing offending foods from your diet.
Diagnosed with ulcerative colitis spring 1999. Have been in remission for years with only a few minor blips.
 
Maintenance dose sulfasalazine. Probiotics, vitamin D, fiber supplement and fish oil caps. George's aloe vera juice and l-glutamine for gut healing. Mostly grain-free and dairy-free low carb diet.

I have been there time and again... I have described it the same way to other people as well - dry heaves out your ass is exactly dead on... I would go through the same during the worst parts of my flare... sometimes from tapering pred, sometimes just because my UC felt like it because its a real prick... Over the years I have found it to be cyclical almost....it comes and goes on its own...sometimes taking months, sometimes weeks....all depends I suppose. Princesa put it just how I would have - reduce stress, anxiety, and take other alternatives like fishoil, flax oil, or probiotics... I think dicyclomine works as well as an anti-spasmodic (Bentyl) - I remember trying that and having some success....
Stats:35 yr old male. UC for 15 years.
location:North Western PA
DX: Left sided UC, most recent scope 5/20/09 UPMC Pittsburgh Presbeterian
Medications:20mg pred (as of 4/27/09), 12x pills Asacol per day, 100mg Imuran per day, 1mg Atavan per day (anxiety) when needed, 20mg paxil per day, Super-omega fish oil pills - 1x per day, Flax oil pills - 1x am, 1x pm, Culturelle probiotics - 2x am, 1x pm, Vitamin B-12 injections weekly.
Surgeries: Recent Endonasal brain surgery to remove non-cancerous hormone producing tumor on pituitary gland (and I am still more afraid of my UC!!)

Oh yeah...butt dry heaves! horrific.

I think you should enquire about rectal meds....

What meds have you been on in the past, or did the doc put you on pred directly?

You could ask the doc for some antiaspamodics...could help somewhat. I use Dicyclomine (Bentylol).

~vitamins/minerals/supplementsProbiotics....(Natural Factors Protec, Primadophilus Reuteri Pearls). @ bedtime
~various digestive enzymes as needed
~URSO for PSC (or PBC) 500mg X 2 daily (LFTs back to NORMAL!!)
My doc's logic.. "TREAT (FROM)BOTH ENDS"  worth it !!!

thanks everyone for replying. I have took it upon myself to up the dosage back to 20mg of Pred again and conditions have imporoved mildly but not completely. I have tried Rectal meds but can't keep them in. I have lost way too much work and lost almost all credibility with friends. It is extremely aggravating when you tell someone you can do something when you are feeling fine and then when the day comes your ass explodes. Gaahhhh!

When I was having the spasms like you describe, my doc said go to 40 for one day, then stay at 30 for a while (I think I did two weeks, then I tapered). I was bummed, but it worked. I'm still trying to shed the moonface and extra weight, more than a month after tapering off completely.
49 year old female attorney, diagnosed UC/pancolitis 1985, no surgery but much suffering. Asacol maintenance for 20+ years; used prednisone & Rowasa for flares.  Probiotics (Ultimate Flora, 50 billion), salmon oil (omega-3) when I can remember. 
August 2008 sought care of naturopathic doctor. Food sensitivity test showed wheat/gluten, other intolerances; eliminated wheat/gluten from my diet.
Bad flare in March, used prednisone. Humira denied, started Remicade 4/12 and doing great~ learning what normal is~ wow

I have done the same as far as self-prescribing higher doses of pred.....lately both of my GI's are adamant about getting off the pred and I am back down to 10mg a day, eventually going to taper off... I noticed that the higher doses of pred really stopped my assplosions and cramps as well - and honestly if I could I would stay on that dose of pred indefinitely - I will take the weight gain and moon face over assheaves (maybe that can be a new UC term?) - but according to my doc's the long term use of high doses of pred is pretty bad....that bums me out.
