Sulfa to Lialda - Made the Switch - Flaring

Just over 2 weeks ago 17 year old son had a blip that might have been the beginning of a flare. Also was dealing with horrible headaches. Doc switches him to Lialda - 2 per day from the sulfasalazine son had been taking for almost 7 years.

Now there is no doubt he is flaring. 5 trips to bathroom per day, loose, blood, urgency. I called doctor and of course he is out of town until Monday. The nurse said he could up the Lialda to 4 per day. Should he do this all at once or take 3 for a couple of days then go to 4 or does it really matter - just double the dose now?

Also, do I understand that when you up to 4 Lialda per day you do 2 am & 2 pm?

Can he up the corta foam to twice per day? or do canasa once per day and corta foam once per day?

The only good news is the headaches have gone away....but at what cost?

I am going to be switching to Apriso as soon as my Asacol runs out, which is similar to Lialda in that it's a once-a-day mesalamine. My doc told me to take all four in the morning. However, you can call your pharmacist, who should be able to answer that question (or perhaps check on the Lialda website).

Honestly, I'd go ahead and up it to four...why take the risk on letting the flare get worse if three doesn't work? You can always wean down later, but if the flare gets bad, it may take a lot to get him back to normal.

That's great the headaches are gone. I don't know if I'd necessairly blame the flare on the switch to Lialda, because of that recent blip. Maybe he would have gone into a flare anyway even if he'd stayed on sulfasalazine.

I don't know what he can/can't do on the cortafoam, as I've never used it. I do think I've heard of people using a suppository in the am and an enema in the pm.

Status: Remission since late May 2009!
Symptoms began in November 2008, ~4 weeks after giving birth to my son.
Eased for ~3 weeks in December, possibly b/c of probiotic use?
Returned in January 2009 (with a vengeance), diagnosed with pancolitis on January 30.
Currently taking Asacol (400mg 4 pills 3x daily), Rowasa twice weekly, Culturelle probiotic, and Zoloft (25 mg).

Thanks, we'll go straight to 4.

I asked the nurse if that meant 2 pills am and 2 pills pm and he said yes. Is this correct? Seems most of my reading says once per day. Does it matter? Which is best?

I prefer 2 dose per day because if for some reason the dose passes through too quickly or whatever, the later dose might have a chance. (We had an issue like this with prednisone passing through too quickly during the horrible morning hours of multiple bathroom visits.)

one gi told me that if you take all four at once, it actually increases the med 7x in you colon.......but they always gave me the option
