Help - Hemmy or UC??

Hi Guys,

I'm not sure what to think here and need some advice....I have been doing well on the Lialda (4x) and nightly Rowasa's (4mg/60ml) and haven't stopped or tapered since being diagnosed on July 20th. I haven't had more than 1 bowel movement since August 16th and haven't had blood since August 7th. Well I did my normal morning routine this morning and when I wiped, I saw bright red blood!! It wasn't excessive, but it wasn't minimal either. I did not eat anything yesterday that I had not eaten before, so I am positive it's not something I ate....any thoughts?

My colonoscopy did show 1 small internal hemorrhoid, but it was non-bleeding at the time (late July) and I didn't notice any blood in the toilet bowl today...

This is quite depressing to say the least...

July 20 isn't that far away, and there are usually some ups and downs, especially if the inflammation and fragile lining is still happening low in the rectum. With a formed stool...it can bleed.

When you use the enema...you could consider to start squeezing a bit of med once you enter and anus..not alot or it will squirt out..lol...and squeeze as you push the nozzle in..squirt most of it and then while withdrawing close to the anus..squirt more med, push it in again and empty the bottle.

That way, you'll have med at the lower part of the rectum and not only in the upper part. Rather than laying on your left side, lay on your back or right side...keeps the med in the area.

Seems you're doing fine....


OK...you should take a look to see if what you have isn't a fissure...it's possible with wiping and a more formed stool.

If there is blood around the stool in the toilet it could be a hemmi or still flare blood....

It takes time, and you're improving A LOt.....HANG TOUGH.

Been there..lots of times.

How do I know if it is or check for fissures?

NiRo - I often have the same question because I have quite a bit of bleeding with UC but I also have internal hemmis that bleed quite often. For me it is sometimes really difficult to tell. You might try some hemmi medication in addition to your UC meds...

Yeah, I am really confused by it all...sorry if this is a little bit TMI, but I just passed gas but had to wipe because there was alot of mucus. I never had that happen to me before. I am not sure if it is from the hemorrhoid, from me being worried about the UC, or from the UC itself. I just gave a call to the dr to see what he has to say, waiting for the call back...

Don't worry about TMI! I don't think that hemorrhoids cause mucus but I have had that happen to me when flaring pretty badly. I would guess you are seeing some UC symptoms.

Spoke to the doctor through his assistant, he said it was normal to have some blood every now and then w/ colitis. I don't know if I like that answer. I have an appointment on Thursday so I will find out more then...

     Mucus is always my first sign.  Sounds like the beginning of a flare to me too.  I don't like the answer you doctor's assistant gave you either.    Was it a brush off?   I hate when they do that.  Good thing you have an appointment this week.
 
Diagnosed with ulcerative proctitis in 1998 in hospital
Hospitalized (2nd time) in May 2008 for ten days.
Remission Nov 08 thru May of 09.
Slowly coming out of flare.  Meds: Colazal (6 per day), prednisone 10 mgm and tapering weekly, Cort enema nightly, Canasa in morning, Benicar 20 mg for BP, flaxseed oil 3 x a day, fishoil 2 x a day, probiotic 3 x a day, calcium with D 1x a day and multivitamin.
Doctor had me on 100 mgm of 6MP which drastically lowered my white count.  Now off the 6MP pending results of repeat white count. UGH.

One thing you should NOT expect is for you to be perfect at only one month since starting your meds.


You have improved....that is good.

You have a bit of mucus and some blood...is that improved from before or not? If it's improved...you're improving,

5ASA meds, both oral and rectal take longer...you want to use them if you can because you can have a nice med regimen for the next 20 years. how do I know? because it's what's worked for me.

Was I where you are concerned I wasn't getting better? Yes, and my doc said, continue to use the meds and it will improve...be patient. I was, it did.

UC takes time to heal...allow it.

Now, if you squat over a mirror, you can see externally what's going on. That too much info for you to know? lol...at least if it's a fissure, you'll see it clearly....or if there's an internal hemmie sticking out (thrombosed)..that'll be obvious as well.

So, hang tough with the UC.

Get the other stuff checked out to see if it's just UC and if there's something else going on, that can also be dealt with.

Mucus will happen with inflammation or not inflammation...but if you have that at this time with blood, I'd say you're still in the healing process.
Now if you have IBS as well...double whammy....

Eventually it'll make sense.

And one last bit.....if the doc suggests for you to go on pred....don't.

I had lots of anxiety and desperation regarding on why I wasn't better.

It sucks.... but I learned about patience and I trusted him.

Hey, I EVEN asked him about pred about 12 years ago...he smiled, said NO....continue using the enemas because you're improving. I did for another month nightly and eventually it wroked.

I'll also suggest one other thing...cut back or nix the fish oil for a while.

Not all will agree, but not all can use it.
