I'm sorry to hear of his pain and lack of response to meds.
I would agree with StealthG to eliminate roughage including all fiber. Perhaps go on a liquid diet for 2-3 days and see if it's the digesting part of the stomach that is causing the pain. I personally would not use Ensure or any off-the-shelf liquid diet because they have so many unnatural things in them. When I used intermittent fasting to give my gut a break last year I used chicken broth, veggie juice (homemeade) and lots and lots of water. Do not allow the hint of dehydration. I avoided fruit juices but that's me. I think he should be able to tell in 2 days if the pain is digestion related or it's something else.
Ugh's idea of looking for an ulcer is a good one and the H. pylori too.
I'm also sorry that SCD didn't help. I've been there too, years ago, tried various diets and supplemets and nothing helped. Nothing in prescript
ion drugs helped either and my flare was non-stopping but without the pain your guy has, just plenty of the rest of the symptoms.
Lastly, has he tried keeping a food journal to see if there is one or more foods that might be causing pain/problems? If he tries the liquid diet and sees improvement that is the time to start a journal and introduce foods one by one and notice any pain/symptoms.

I just saw this post in another thread by BabeintheWoodsOne of my UC symptoms is stomach pain that can be severe when in a flare. I found that chewing DGL licorice 20 minutes before meals and before bedtime to really help.
Presumably if it helps the pain then perhaps it is helping to repair tissue? DGL must be chewed to interact with the saliva. It is hard but not impossible to find DGL without other crappy additives (to make it taste good I suppose) that don't help me.
