Do you trust and have a good relationship with your GI? If so, call him and tell him you'd like to try some rectal meds like canasa suppositories. If that doesn't work after 10 days, you can always go back to him and ask him for enemas. If you don't like him/her, then can you get a recommendation of another GI who you can work with, perhaps through the CCFA?
Honestly I don't think the probiotics you have are strong enough, sorry. VSL#3 probiotics, very expensive and very effective, are especially made for Ulcerative Colitis, with 450 billion active cultures - so that's a huge difference! But here too, if you have good insurance coverage, and I hope you do (I don't), you can get your GI to prescribe VSL #3 DS (double strength) and you may only have a small co-pay.
Here's the link for the Dr. Mercola's Pro-Optimal Whey protein: http://proteinpowder.mercola.com/Pro-Optimal-Whey-Protein.html . My favorite flavour is chocolate, but he has others as well.
With regard to multi-vitamins, most are hard to digest and hard on the gut. I use liquid vitamins and minerals, which I add to my shake daily. Here's the link for these:
The "cocktail" shake I've developed has come from lots of research and trial and error. It's delicious, and I also take comfort in knowing I get all the nutrition I need in it as well. But it works for me, and I hope it will work for you too.
L-Glutamine powder I get from: http://www.nutrabio.com/Products/glutamine1.htm , each scoop is 5grams, so I put 3 scoops in each shake.
Actually I got the recipe for this delicious shake is here: http://cureforulcerativecolitis.com/morning-glory-shake
With regard to Vitamin D3 deficiency, it's really important to get blood work for that, because if you don't have a deficiency and you are supplementing with D3, it could lead to some toxicity.
5-Loxin and Chia seeds you can get through Amazon.com - IMO you should take at least 500 mg of 5-Loxin (but you need to read the precautions and interactions with other drugs you may be on), chia seeds should be organic kind.
Let me know how else I can help you.
Have you given any thought to looking into LDN yet?
