If you're not having any improvement with lots of urgency from a month of use...definitely ask the doc for you to do stool samples. You very well may have some sort of gastro invader.
I would suggest, only for a short while to see if it helps, to stop the flax/fish/borage, etc oils. Sometimes that can loosen and create some urgency because it's very slippery.
How are you eating? There are many on here who are advocates of the SCD and claim success...maybe that's something that you should look into. Besides, I think it would be of benefit to your diabetes type 11.
Maybe use the mesalamine enemas at night and use the cortifoam during the day??
