Yeah, some small part is systemic and can cause some issues. BUT many take oral 4.8 plus rectal mesalamines too. This makes me wonder if the mesalamine max dose is more connected to oral rather than anything to do with rectal. Patients taking rectal mesalamines retain them for many different time frames. Is there really much differnce in a patient retaining a rectal med for eight hours or patient #2 retaining a rectal med for 4 hours and then immediately doing another rectal med of the same dosage for another 4 hours (of course there would still be some residual left over from the first time)?
Also, let's say there is that 4.8 gram limit for just oral mesalamine. Why couldn't a patient take 2.4 grams each of two different mesalamines to get more coverage throughout the tract instead of all 4.8 grams all dissolving in roughly the same
location? Let's ignore the fact that some insurances might not cover two different mesalamines at the same time.
