You really need a scope to tell for sure. I believe urgency is usually lower down and cramping higher up. Fresh, bright red blood is usually lower. Tenesmus/Rectal pressure is lower. I think a lot of symptoms are more general. If you had very limited rectal inflammation, you could have almost no symptoms. I had proctitis that was mild and all I had was a little blood on the TP. I've also had proctitis that was severe and I had every awful symptom possible.
