Hey all, long time no see. smile My partner and I have been discussing starting a family and I was wondering about the folic acid requirements while taking azathioprine. I've heard we need more than the average woman does and I was wondering how much was recommended to you? And when did you start taking it?

Any other advice is also appreciated. I've already been told I need to see a high risk ob-gyn and I'm going to ask for a recommendation from my GI. Questions I should ask my GI or gyno? What were you told regarding breast feeding? I've read mixed things on that.

Here's a few good resources. A note on feeding "Azathioprine and 6-MP are can be continued during breastfeeding; the first 4 h of breastmilk after consumption may be discarded to minimize the amount of drug transferred to the neonate"

I just watched a great video on pregnancy and ibd from ccfa. Here's the link.

https://m.facebook.com/story.php?story_fbid=1188909577861585&id=101393913279829

They do say that new research shows imuran/6 mp are ok to be pregnant on and breastfeed. And go further to say that yes even though the highest concentration of the drug is in your system at 4 hours that it's not worth pumping and dumping because the amount is so minuscule.

This video made me feel so much better. I already have 2 kids. First I got my symptoms and diagnosed after. Second flared 3rd trimester just like first. Now I have a pattern. I flare when estrogen rises pre and post partum so if I want a third my ob and gi will be proactive with me.

Please feel free to ask me any questions. A lot of women sail thru pregnancy when they have auto immune diseases. Good luck!!!!

Thanks guys. I really liked the first article. I'll try to get to the video after work today. smile It's nice to see that breatfeeding may be an option! I thought I'd never be able to.

Still wondering about the folic acid. I'm trying to be ready now in case any accidents happen! Lol

Do you have a MTHFR gene mutation? It's somewhat common in IBD patients. I ask as that gene mutation impacts your folate (vitamin b12) levels. There's two forms of that mutation, a mild form and a more severe form. I know i have the mild form, which makes my body only 66 percent as capable of processing folate as those without any gene mutation. The more severe form makes your body only 33 percent as capable as those without a gene mutation. I had my dna analyzed by 23andme and promethease for info on the MTHFR gene and others.

I have no idea. Could my doctor tell me? I just heard we need more folate because of the meds. I didn't know about the gene

I hadn't heard the drug impacts folate levels. Genetic testing was my way of finding out. Not exactly a huge concern for most, but an interesting footnote on why your folate levels might always be a bit low.

As far as I know they aren't low, but I don't think they've ever been tested.
