I had this same question awhile back and was told they can reach the splenic feature (where the descending colon and transverse colon meet).
F, 51
2008 scope dx: acute proctitis
2017 scope dx: chronic UC (rectum & sigmoid)

Currently taking:
Pentasa, Rowasa
Advanced gut health probiotics, Primadophilis Reuteri, Benefiber, Digestive Enzymes
Avoiding sugar, processed foods & raw veggies.

Extras :
Ginger, vitamin C , vitamin D, cal/mag, vitamin K complex,

Petah, I just noticed in your signature that your enemas are one gram. Mine are actually 4 grams.....and my inflammation does indeed reach higher than the rectum.

Your sig also says proctitis....so maybe yours is confined to rectum? in any case, I have to retract my original post because maybe one gram wouldn't go further up?

I just follow the directions and lay on my left side for 20 minutes after insertion. Maybe someone else will pipe in with more conclusive info?

Hi marji.. just came from ed and they done a ct scan which showed inflammation in the defending colon. I've just increased to 2 enemas a day until I see my GI..no mention of recital inflammation which has me confused..my inflammation has only ever been 8cm rectal

Hmm.....well 8cm rectal = 3 inches......so a one gram enema should take care of that?

As far as your descending colon involvement, I am not sure. I'd think a larger volume of fluid would be necessary to reach the splenic feature (as I mentioned in my initial post). But I am new here, too....and learning as I go. So as others comment to you, I will also learn.

Thanks for your reply and help. I'm not sure why it's spread unless the inflammation isn't from my IBD

My GI also told me 2x 1g enema is for a flare not mantinence

Hey there, ok so this study says that in 7 out of 8 male participants the 1 gram enema did spread to the splenic feature. So it sounds like your inflammation should be covered?

As far as spreading and disease depth goes, I am clueless. Mine has spread, too some.....this may be the nature of the beast. And I guess doing what we're doing with keeping it treated and inflammation as low as possible is a good course of action. That's my personal plan anyway. good luck.

I am currently experiencing a break in symptoms (clinical remission) but still have inflammation, so technically I am still flaring and my enemas and oral meds are treating that.

Ok thanks. I guess I'll know a little more tomorrow when I contact my GI.. I've put my enemas up to two per day and the fluid definitely stopped the pain so hopefully I'll sleep without being anxious. Thanks again

You're best with a higher dosage of enema such as 4g. But using the 1g twice daily is better than once. Please ask your doc to get you on an oral mesalamine....if your UC is climbing, you need better coverage.

It's not guaranteed it will reach splenic flexure so don't make that a determining factor for your only med. It definitely willmgo past the sigmoid.

Keep in mind there are Pentasa enemas with 100ml liquid and the other mesalamines such as Salofalk, Rowasa and generic that are 60ml liquid. Foam enemas are available in the UK for sure, 1g dosage, and I wouldn't state it reaches the splenic flexure.

Were you completely off the enemas before this?

The amount of liquid determines how far it spreads. If it's at least 60ml then it should reach the splenic flexure.

I belive red kidney beans caused my flare. I was using the enema at night. Since getting the fluids last night I feel so much better. I will definitely discuss the 4g enema but I they might not be available in Australia. The one I'm using is 100ml
