Colonoscopy results-some good news

The short of things is that my UC is responding to something! My GI said a month ago it would have probably looked a lot worse. Now to get the symptoms better....

Findings:
The terminal ileum appeared normal.
Inflammation characterized by congestion (edema), erosions, erythema,
friability, granularity, loss of vascularity and mucus was found in a
continuous and circumferential pattern from the anus to the splenic
flexure up to 45 cm. This was moderate in severity. Biopsies were taken
with a cold forceps for histology.
In the right colon there were patches of purulence with erythema
scattered amidst normal appearing mucosa. Biopsies were taken with a

He ended up last minute deciding to do an upper scope too. I have a mild hiatal hearnia and LA grade B esophagitis. Also Diffuse mildly erythematous mucosa without bleeding was found in the
entire examined stomach.
36yr old mother of 3 Moderate Pancolitis 1/3/17. Hosp 1 week on Solumedrol, Delzicol. Apriso, pred, then switched to Balsazide 750 mg, and Uceris 9 mg 3 wks. Minor flare in August, flare since Oct 20. Acute pancreatitis from Balsalazide hosp. 2 days, Started Humira 12/15/17, reaction, Entyvio 1/12/18. 40 mg of Pred, still waiting for a change!

Pretty miserable. Two hours after eating, d started up with a side of extra pain to go with it.
36yr old mother of 3 Moderate Pancolitis 1/3/17. Hosp 1 week on Solumedrol, Delzicol. Apriso, pred, then switched to Balsazide 750 mg, and Uceris 9 mg 3 wks. Minor flare in August, flare since Oct 20. Acute pancreatitis from Balsalazide hosp. 2 days, Started Humira 12/15/17, reaction, Entyvio 1/12/18. 40 mg of Pred, still waiting for a change!

Wow that sounds miserable! I hope you get to feeling better soon!!
Diagnosed at 7, in the gray area between crohns and UC. Surgery on Jan 27, 2018 #nocolonstillrollin

Glad you're responding to something. What you describe in the report is still considered to be flare signs but of a moderate intensity. And moderate is better than severe. Do you feel the entyvio is beginning to kick in?

Feeling a bit better this morning pain wise. I am hopeful the Entyvio is kicking in.

Could be the first small step in what will become much bigger improvements with Entyvio. *fingers-crossed*

What did your doctor say about the inflammation of the esophagus and stomach? Any plans for treating it? Any mention of Crohn's or is there something else going on elsewhere within your digestive tract that explains it?

I wonder if you have h. Pylori....did the doc take biopsies there as well?

He did take some biopsies. He prescribed omperazole, but I have not filled it yet. I really don't want to take it, (will if I have to) so I have been looking at other stuff. Ran accross some info about taking digestive enzymes, but still researching. I would like more info as to what is causing the issue, but I guess I have to be patient and wait for the biopsies. I understand Prednisone can increase acid, but I think I was having issues before I was on pred.
I was wondering about the h Pylori as well. Do you ask about Crohns because of the esophagus or the patchy in the colon or both? Can one have diagnosed pancolitis at one point and it be Crohns? I don't really understand the cases that switch.

He wanted to prescribe mesalamines enemas as he said they would greatly reduce my symptoms. He was super frustrated that I can't have them. He said he was going to do some research.

Patchiness and esophagas involvement always ask the question "is it Crohn's disease?," in my mind. The answer might very well be No, but it cannot hurt to ask your doctor. It's rare to switch between types of IBD but does happen here-and-there when something like aphthous ulcers (a unique crohn's thing) or other similar things unique to crohn's pops up. *shrug* I'm no doctor of course lol.

Hi Spring

I was also wondering if you had Crohns due to patchiness. I had patchiness as well and doc was questioning Crohn's colitis instead of UC. How does prednisone affect biopsies? When I had my colonoscopy on 4mg of prednisone someone commented that my results couldn't be trusted due to the prednisone. Now that I am off prednisone everytime I have pain or a loose BM I am thinking the inflammation is coming back. I am glad your results were moderate and not severe!

diagnosed proctitis April 2016 s/p a c diff infection age 53
Nov 2017- ?Crohns colitis to mid transverse - mild/moderate
remission March-Sept 2017 then flared again-failed prednisone wean
started Humira 11/17/17, off prednisone since 12/26, balsalazide 3/ 3x day cannot tolerate Lialda or probiotics
past meds- cortifoam, canasa, rowasa, plan is -will be off all meds except Humira

It will be interesting what the biopsies come back as. When the doc talked to me he said he thought it had been healing in that area. I have been on steroids for almost 4 months now, so if that is true, then it would be skewed. I am being optimistic that the Entyvio is starting to reduce the inflamation.

When we're on pred, the inflammation is less than it otherwise would be without it, and that's all they are referring to. The biopsies don't lie, and aren't altered (other than showing less inflammation) when we're on a pred and when we are not.

"I had patchiness once" is a good thing to put on your backburner and keep in mind for the future. The majority of your data points and history point to a UC, and it would take multiple strong data points to change that direction -- more than just one iffy point.
