I would consider a switch to prednisone if you're dealing with debilitating symptoms. It has a lot of side effects, but it can work quickly and provide relief when you're in a bad place. There were times I was going upwards of 20x a day with pain, cramps, blood... and it got me feeling artificially well nearly over night! I say artificially because pred makes me crazy. I would have WAY too much energy on it, which was usually somewhat welcomed in the beginning since I was so weak and fatigued leading into accepting that it was the answer. As for the Apriso, if you're at the max, and it's not helping, I would consider a switch to another. It may not work, and I wouldn't put all my eggs in that basket, but it's something that's easy enough to try. I also wouldn't discount rectal meds just because you have pancolitis. Even if you got some relief in the rectum, it would be beneficial to your overall condition. Have you had stool tests to check for inflammation and pathogens? If you're not improving, it can always be a pathogen that's not getting treated, but in my case it never was, it was pancolitis rearing it's very ugly head.
