No drug is definitely safe it is up to a user to decide whether the possible side effects are worth the efficacy displayed in the available data. The people who truely know all the interworking of biologics are all PHD level education with vast years of research. Most good IBD doctors will know the broader picture of the drugs mechanism of action while many GIs will have no clue. I don't see there being any long term side effects that wouldn't already be displayed in the short term. At most we may see them pop up at a higher rate after extensive long termuse, but I don't thing that is likely. The drugs are blocking a single signaling molecule in our immune system so their harm is limited to some consequence as a result of our weaked immune system or by our body eventually forming antibodies to the drug disperesed throughout our body.
