There can sometimes be a disconnect between symptoms and inflammation. I thought I was doing well (I had mild urgency, 4-6 bms a day), doctor did sigmoidoscopy and said I looked like raw hamburger meat inside. He said surgery or remicade, my choice. Chose remicade and been doing well ever since.
If you think about the frog being slowly boiled in a pot of water, and not knowing what is happening. Same thing as our quality of life slowly deteriates, we adapt to it, and do not know how bad we actually are doing.
That said, Calprotectin stool test or C-Reactive Protein blood test would also pick that up. Colonoscopy isn't essential in proving bad Inflammation is lingering.
