Hey Hugo,

Thanks for replying on the other thread. As mentioned I’m a triathlete too. I’m newly diagnosed and lost a ton of weight during this first ever flare. I’m tapering prednisone, 60mg when I got home from hospital, tapered 10mg per week until 20mg and then 5mg per week until done. I’m currently on 5mg and will be done next Tuesday. Symptoms - 2-3 BMs per day never formed. Just started Humira as Inflectra didn’t work. Had my second loading dose of Humira this last Wednesday. I lost 21kg and a ton of muscle, put on 10kg since returning home. Still feeling pretty tired overall.

Some Qs:

1. Do you know if prednisone could or does increase risk of injury?
2. I seldom have cramps but still have discomfort now and then. Do you think I can start running short distance now or should I wait until I’m done with the prednisone?
3. I’ve got on the bike 3 times indoors. Just peddling at 30% of old FTP. Do you think low cadence strength would be better versus longer endurance type rides?
4. Do you have any tips to get back into running?
5. Do you have any BM issues while running?

If noticed my resting HR is heading back to normal ranges but my HRV continues to show a value that’s indicative of stress. Just some extra info.

1 Yes, you can tear ligaments. Pred weakens ligiments. One person tore one weight lifting while on pred. Pred slows the rate your body heals, so a light workout will wreck you the next morning v still sore and achy.

2 You can try short, easy jogs and see how you do. I wouldn't push personal bests on pred.

3 Stick with low cadence.

4 Take it slow and easy. You've lost a lot of muscle mass and will feel weak.

5 Depends on the flare. Certainly choose routes you can take bathroom breaks during, if needed. If you're worried then use the treadmill or elliptical machine at your local gym.

When I was flaring it was hard to run without going to the bathroom while in a run. It is almost impossible to do. In fact I moved to a treadmill for my runs during flaring.
I find cycling way better. Just stick in zone 2 at a lower HR for now. When I got back from my first bad flare I think I ramped up way to quick. Remember, intense exercise brings on inflammation and that is not what your really need right now. I did a massive Fondo 4 weeks after my first recovery for a flair and my CRP jumped specifically from the ride as I we measured it a day later and then 3 days after that. Stay easy and low intensity and then you can build up your time on the bike!

On a tangent but what was the reason inflectra didn't work? If it was antibody formation then you stand a good chance with humira. If it was just that you had no response despite therapeutic levels of the drug in your body, you may do better with a different class of drug...(since inflectra and humira are both tnf inhibitors)
You have to be careful with your bones on pred too (depending on how long you took it for and at what dose) as there may be osteoporosis. A dexa scan will reveal if you have bone loss. There was a girl on here who liked to run and fractured her foot which she suspected was due to prednisone.

my advice would be to find a nice comfy chair and chill -

give your body time to heal - it will thank you later -

unless of course you're 90 - in that case, forget what i just said !

@iPoop & @Hugo - Thank you, I'll stick to the low cadence Z2 stuff for now on the bike. I had my last prednisone this morning (5mg). I'll have a look to see if I can find info on how long it stays in your system.

@TroubledTurds - It makes sense to rest, I'm not trying to be unreasonable, I just need to move a little and help my body get back into movement.

@poopydoop - I was forming antibodies and had a reaction during my second infusion. I did feel like it was working though. My doctor said I could try one more infusion but since I was forming antibodies and had a reaction, I was most likely going to react again. She felt that my body was metabolizing the inflectra too quickly too. I feel like the Humira is working, I've only had two doses (1 x 4 injections and 1 x 2 injections) my symptoms aren't improving much but I think it's because of my diet. I'm still flaring and have introduced salads and vegetables, I was just getting super tired of the regular bland stuff. My thinking was that I could put up with a little more pain and discomfort while adding more variety to my diet. I belive that my diet is not going to slow down recovery, it only makes symptoms worse. I'm still learning so may have misunderstood that part.

So, an update! I decided to go for a very short and very slow run yesterday, it was super humbling. All I could manage was 1.25km overall in just over 10min. I was running 3min @ ~ 7:45/km and 1 min walk. I didn't experience any urgency or pain in my abdomen, I did have a tiny bit of discomfort though. My legs were hurting quite a bit and my lungs felt like they hadn't been used in months. I was super surprised how much my lungs were burning. It's really highlighted how patient I'm going to have to be to work back to where I was.

Some people need it to be proven, although many take your effects as meaning I need to keep at it....to their denied detriment.

fortunately I’ve had to be super patient in the past as I’ve spent months and months training for certain events and also had to work wisely through injuries. It’s good to know what the current baseline is But I heard your recommendation, Quincy. I’m not going to push it.
