Hi I’ve posted about this before but just have a general question for everyone. For those on imuran what’s the dosage you’re taking? My doc had me on 50 mg along w lialda but it’s not working - every time I try to taper off the hydrocortisone enemas I flare again. Has 50mg worked for anyone??

I think I started on 50mg, then 75, then finally 100mg/day for the last ~10 years, which has kept me out of a flare for 5+ years.

I weigh around 180 pounds. People on here tell me that's still a lower dose for my weight.

Over the last couple years I have been prone to bad infections, sinus infections, staph infections in skin, and a case of shingles while in my mid 30s. But not having flares makes that bearable so far.

There is a blood test for imuran to test if it's within therapeutic range in your body. If not, you can increase the dose. 50mg is a low dose unless you're using it in conjunction with allopurinol.

Azathiopurine is a weight based drug, so impossible to answer your dose without knowing it.

Basic formula is be within the range of 2.0 to 2.5 mgs of azathiopurine for every kilogram of body weight. For example if you weigh 150 pounds (70 kilograms) then it'd be:

2.0 mgs x 70 kgs= 140 mgs of aza per day

2.5 mgs x 70 kgs = 175 mgs of aza per day

The second important dosing factor is your bloodwork. Some cannot safely get to the above mentioned dosing without inadvertently getting Hepatotoxicity. So they're gasteroenterologist keeps them at a lower dosage for safety.

50 mgs of azathiopurine daily isn't within that range unless you're concurrently on allopurinol due to Hepatotoxicity.

I'm on 150mg (I weigh 172lbs or so) and have been for about 4 years now. A couple months ago my liver enzymes came back super high with my bloodwork so my GI wanted me to stop drinking (hey it was a long quarantine okay) for a month and get tests again. I have bloodwork on Wednesday and I'm hoping my levels are back to normal and I'm not going to have to lower my AZA prescription because I'm in a really good place with my UC right now. Fingers crossed!

I weigh 100 pounds and take 100 mg of Imuran a day. I get regular blood work to make sure that my liver function is good and that my white blood cell count is in the normal range. The accepted does to treat uc is 1.5-2.5 mg per kg. I started at 50 mg for a few weeks then went up to 100mg. I also had a TPMT done before I started taking it to make sure I was a good candidate for the drug.
