Hi smile

I have a moderate to severe case of Proctitis (20cm) affected.

I’ve been prescribed Salofalk enemas 2g one in the evening & Salofalk granules 3g once in the morning.

I’ve been taking them for 5 days and not seeing any improvement - is that normal? How long do they take to kick in usually?

My main problem is bleeding and a horrible gurgling colon! I usually have 3 BMS a day but 2 of them are usually bloody mucus (the joys!)

Any advice would be much appreciated.

Patience. Mesalamine is a mild anti-inflammatory medicine that might take 2-4 weeks to begin showing an affect. It might take 1-3 months to feel normal again. Some trial-and-error in treating us, so possible that mesalamine isn't enough for a moderate case.

Thanks so much @ipoop! Trying to get my head around everything and have so many questions so thanks for taking time to answer.

Due to me being anaemic I’m quite anxious for the blood to stop but I know it will take some time.

If you don't get an initial response within a few weeks time, things get worse, or you're suffering too much, then you might need a stronger medicine. Often a form of corticosteroid is used in oral pill, rectal foam, enema, or suppositories. They're usually prednisone/prednisolone, budesonide, or hydrocortisone. Ask your gasteroenterologist in that case.

Steroids are rocketship fast in relieving our suffering. Whereas mesalamine can take weeks.

Do you have 2g or 4g Salofalk enemas? If you have suppositories, use one in the daytime after the majority of your bms.

Good you're on an oral mesalamine as well.

Are you taking an iron supplement?

Thanks so much @ipoop! I have a follow up appointment with my GI in a few weeks, if things have not improved I will certainly speak to him about the above treatments you’ve mentioned. Thanks again 😊

Hello @quincy 😊 I am currently taking a 2g Salofalk enema (just realised I made a mistake in my first question!) no suppository. When I was initially diagnosed I was put on a 1g Salofalk suppository which didn’t do anything in afraid.

I am taking liquid iron as prescribed by my doctor as she didn’t want to prescribe me tablets due to my Proctitis. Have been taking it for 3 weeks and having my blood checked regularly. Thank you for your help also - so appreciated!

Hi both,

Hope you are doing ok!! 😊

So, have been on them for a week or so now and am not sure if they are agreeing with me or not!

I am going once or twice a day, usually the first stool is very loose, is that normal? I’ve not really had loose stools before!

Bleeding is somewhat better as not going 3/4 times a day to expel blood/mucus.

I must say I do have a lot of gurgling & sometimes gas.

Going to Stick with them and see what the next week brings before my GI app.

Should I be seeing more improvements or is this normal?

You arent getting worse so they are doing their thing.
See how you are at the one month...yuu will probably need longer.

Pay very close attention as to how you are healing rather than being impatient.
I've done mesalamine oral/rectal coming up 32 years...patience is what you need...trust in the process. Plus you can ask for increase dosage enemas of 4g.

Is the fact I’m having looser stools ok? Usually have been more solid but am thinking it’s just the meds.....

Loose stools but the same frequency?
Looser can mean your inflammation is higher but still mild, or yes, from the enemas at this point with inflammation and more liquid in the lower colon and rectum. It could also be from food choices that would increase motility or loosen stool. But since you're flaring and not getting worse, just note symptoms and continue what youre doing.

Hey Q!

Hope you’re having a great weekend!

Just thought I’d update you. Unfortunately I’ve had a rough time. My symptoms persisted and turned into watery diarrhoea, more blood and stomach pain.

Meting with my GI tomorrow to see what’s going on..... have had to come off the medication for now to see if it’s an intolerance!

Feeling pretty down and hope I can get something that works for me soon sad

Hi...good you're seeing your GI. Request a stool sample kit for invaders. Also enquire if you should get checked for Covid.
If something else is going on, it's not the meds and they won't be doing what they should be doing.
