Entyvio and Gout

Hi All,
Just checking - Anyone in Entyvio experienced Gout?
DD is on Entyvio from June. She is been doing so good and gained back almost all lost weight.
Her iron level went up but WBC is still slightly elevated .
GI said to wait for 3months and if still shows as elevated have to check with Rheumatologist.

Right now DD is dealing with Gout kind of symptoms on her left Toe. Her toe naturally is little bended inward with a bony outside .So we thought it could be Bunions.
Took good care of it but even after 1week the symptoms of swelling/ slight pain remains.
Primary care said it seems like Gout. She has to get her Uric acid check today.
Physician prescribed Perdnisone short course for 1week as naproxene is not favorable for UC.

I did some googling and found here and there Entyvio /gout relation.
All of them are 50+ age group except one .
My daughter is 27yrs so wondering, as they say gout is mainly for men and for women it is 50+ age group.

Any tips/ suggestions/ advise please share..

Hi Bloom93, sorry to hear about your daughter. I cannot answer your question about Entyvio & gout being related. Hopefully she will know for sure once the lab work comes back. If the test comes back negative, ask to be referred to rheumatologist.

Gout does typically hit men more than women, however, women can get it too. My husband had it in his late 20's & his diet was pretty much the culprit. He changed up his eating habits & was never bothered again. The Mayo Clinic's website has excellent detailed information about gout, you may want to read there.

Bloom, I'm not surprised that it can cause gout. Entyvio was formerly a chemo drug and then got repurposed for UC. I know 2 people (one family member) who developed gout after using a biologic for cancer (not Entyvio). Entyvio is still new so they don't know the LT effects.

Online there seem to be recommended diets for Gout.

What made her stop SCD? Did she hit a wall with the diet?

Glad entyvio is helping her. Well done.

It's extremely unlikely entyvio is causing inflammation in her big toe. Entyvio binds to a specific integrin which is expressed on a small subset of T-lymphocytes. Those lymphocytes prefer to migrate to the GI tract. Hence it is unlikely to cause those symptoms.

Could be a result of the slightly bent toe itself. Hope it settles.

Higher level medications always scares me hence the doubt !


@ Jane974 - Regarding SCD - She followed it strictly for close to 2yrs.
In between she had home FMT which puts her into remission quickly. Because of this unexpected quick disappearance of symptoms , we thought her UC diagnosis was a mistake and she slowly got back into normal diet.

I would try FMT again (if needed to go off biologic) given that it put her into remission quickly.

I’ve been on entyvio for 3 1/2 years and don’t have gout symptoms. I do experience some tenderness in my feet when I first get up but it goes away after I walk a bit. but not sure if that has to do with entyvio, UC, or age.

@ Jane974- She had an FMT via colonoscopy this 2nd flare -but it did not result in remission. May be because the stool was from common pool. The first time we used her brother's.

Update -
DD Uric acid test came back normal at 4.9.
Primary care suggested to take prednisone and it is helping her and is almost back to normal.
