Failing Humira

Hello all,

Week 9 of Humira with almost no real improvement, still urgency / frequency. Blood and D had stopped until step down from 20 to 10 of pred, then blood returned, no D though..... (rapid taper on advice of endocrinologist as now developed Cushing's).

Understand can take up to 12 weeks to see improvement, but we are at 9 here.....barring a miracle.

Do have some good days, but most are still typical flare days, some (like today!) just all day like a clean out day. Cannot leave the house. Lots of cramping and almost constipated (different than usual tenesmus. Heading in for first blood work / antibody check / next week.

What's been people's experience (WE ARE ALL DIFFFERENT AND RESPOND DIFFERENTLY), but have most of you - failing one biologic - tried another like Remicade or Entyvio?

Was so hopeful but really no meaningful change here.

Did you drop from 20-10 with nothing in between with pred? That could be the problem. When I go below 20 I have to drop by 2.5 mg to keep the blood at bay.

I would give it more time, if I remember correctly you had some lag time before getting it approved. I know you said on the 8th you were doing some better, maybe you dropped the prednisone too fast. Are you on any rectal meds? I would see what the labs show, another option is to do weekly injections, this is not uncommon. It can be a bit of a hassle with insurance, but if the drs office staff is good they can get it done. I feel for you, when we feel bad one week can feel like forever!

Nothing in between 20 to 10 drop, again, on advice of Endocrinologist. But popped back up to 15 and will taper at 5 or 2.5 per per instruction. I would think at this point in Humira, and still on Lialda, the pred taper would be a little less dramatic.

The weight gain / Cushing's is extraordinary this time around. I can barely see when I smile , my cheeks are so huge!!!

Yes, every other day steroid enema.

Just really haven't seen overall improvement one would hope / expect. As usual, curious on other's experience. Look forward to hearing more.

I failed Humira but had success with Remicade. I did have a response with Humira for awhile but not long . I guess I lost response but didn’t fail it altogether. I wouldn’t waste a lot of time with it if you’d had no response . You could try Remicade or a drug with a different mechanism.
