A question for Xeljanzers. I have been in remission for over a year on 5mg daily twice on xeljanz. 10 days ago symptoms such as slight bleeding and urgency has arisen. I pumped it back up to 10mg twice a day. How long do you think I need to find results from the increased uptake? I hope this isn’t the end for me. I was doing so well!

Have you tried rectal meds as an adjunct? They can work faster.

I added rectal mesalazine (salofalk) when I started to flare on 5mg, rather than upping to 10mg. I felt better almost immediately.

I take 10 mg in a morning and 5 mg at night which gives me the right balance that works for me.
My specialist is fine with that.

I started rectal last night. Did you use supp or enema? The only reason I didn’t right away is that they didn’t help me in my last flair. Did you go up to 10mg Poopydoop?

Hi i used the enema at 4g strength. Interestingly the enemas never worked for me in the past; I think once the inflammation reaches a certain level then it's like using a water pistol to put out a house fire. But because i caught the flare early then it helped.

My doctor only wanted me to go back up to 10mg as a last resort because I'm on birth control pill and that combined with xeljanz may put me at an elevated risk of thrombosis. So I've stayed on 5mg. However I've been afraid to wean off the enemas completely (i still use them a few times a week) as I'm afraid I would flare again. And I've been doing this for 15 months.

I used the enema yesterday and everything was better today. I think i will stick with the 10mg and the enema. It can't hurt to use. I just felt perfect on the 5mg, so a flare was really surprising. My remicade never was as good as the xeljanz.

Hi good to hear! Hope you continue to improve. The flare might have been building for a while before you had symptoms. Like you, nothing has worked for me as well as xeljanz, but I also never felt perfect on the 5mg dose until I added the enemas.
