I have been on xeljanz for almost 2 years. It worked incredibly well for me. The last month I have had small symptoms. I had a scope 2 weeks ago and showed inflammation 2cm from the rectum. Felt great after the scope but the last 5 days I have had mucous and some blood. I have been on pentasa supps and enema and went on a cortisone cream the last few days.
When I got really sick 4 years ago steroids didn’t really work. When I get sick I really ramp up. I know there aren’t a huge amount of options for me but should I stay the course or move onto Stelera? The good news is that I react well to new biologic. The bad news is they only last for a couple years.
Thoughts?

Hmm that's a difficult decision. I'm assuming you're already taking the 2x10mg dose of xeljanz so increasing is not an option? There is entyvio as well as stelara. (Also what was the reason for stopping remicade? Did you have antibodies, in which case another tng inhibitor might work, or just a lack of response to a therapeutic dose? )

Remicade just stopped being effective and inflammation arose. Yes I am on 10mg twice daily. My options are dwindling. Stelera, entiviyo or perhaps the new MS based drugs. I will stick with the xeljanz and see if the rectal meds can get it under control. In January when it acted up I was able to control it but in the last week it is way more intense with the mucous. I am wondering if i could take the xeljanz combined with the stelera for a bit..

Now my GI says he wants to use tacrolimus enemas to assist me with my proctitis. Has anyone heard to this before?

These tacrolimus enemas are not easy to compound. It could take a week for them to be produced. Neither the steroid enemas or asa 5 are doing a thing. In fact my proctitis is getting worse by the day. Really, really depressed. I hate when the GI tells me sorry but your opportunities are starting to run out. Xeljanz was so darn good for 18 months. Major stress and boom I am zero to 100..
