Hi all,

I’ve been dealing with anal fissures on and off for the past year. My doctor had originally recommend that I increase my fiber intake, which I did but it only seemed to help a little. He then recommended a cream, which only helped at the start. I was wondering if anyone else has had trouble with anal fissures? My UC is under control, and my last flare was about two years ago. I had a colonoscopy back in June and everything was normal. I am meeting a colorectal doctor on Monday to discuss next steps. I am definitely worried and hate seeing blood every time I wipe. Does anyone have any recommendations as to how they recovered from anal fissures. I definitely don’t want to have surgery for this.

Hello,
I have had the same problem for several years while in remission.
Initially my doctor gave me a cream, cannot remember the name but the main ingredient was Glyceryl trinitrate. It was useless and gave me the worst headaches of my life. I went through surgery and the consultant was supposed to inject some botox as a way to relax the muscle. Once I was under the knife he could not find any fissure. The b******* came back a few months later but at that point I felt very disheartened and did not want to consider going through the same procedure again. I spoke to a doctor back in Italy when I was there for summer and he recommended to me another cream called Levorag. That worked like a charm. Also on another forum i follow many people swear by proktis-M but I did not try that one.

I had anal fissures for a year before my UC first developed. I tried everything. The only thing that eventually worked was using lubricating suppositories right before every single BM, and then medicated cream right after every BM. Also, never push or strain to get stool out, let it come out naturally. If you can't get it out without pushing a lot, then your diet and water intake are off somehow.

It took me 3-6 months to heal it this way. It was always 1 step forward 2 steps back.

In my non-medical opinion, fissures are still part of the UC picture, even if scopes and tests show things are under control. Most UCers have the worst problems in the last 12cm of their colon.

Vanjordan, I definitely feel like I’m always taking one step forward and two steps back. Every time I think the cream is working and healing the fissure, I end up having a really bad BM. I’m hoping that he will recommend another cream or something...

I feel your pain. With fissures you have to be really disciplined in thinking about everything mechanically. Like, is this food I'm eating going to cause larger or harder stool? Is it going to cause a lot of friction or irritation on the other end? Am I pushing too hard? Did I lubricate well enough before my BM and topically treat enough after? Am I pushing a lot?

I made goldenseal suppositories with cocoa butter after over a year of trying modern nitro creams. They worked. I'm not a fan of suppositories in general but I was desperate. Goldenseal heals the intestinal epithelium pretty fast. You're still 1 step forward 2 steps back but because they speed healing, the merry go round ended faster for me. You might want to consider herbal suppositories. Herbs actually heal the body, the modern creams don't. Or you can use both.
