The bleeding just won’t go away…

So I’ve been in a flare now for over a year. During this time I’ve been bleeding almost every BM.

Last scope about 4 months ago showed 4cm moderate inflammation and I kept on the oral and rectal meds.

Calprotectin was 3000 about 2 months ago, then 1000 a week later. I think I caught mucus on the first.

Ive been on salofalk enemas last year, foam the start of the year. For the last 4 months Salofalk suppositories morning and night.

For the last 30 days I have switched the morning suppository to budesonide foam.

All of this and I still bleed. It happens towards the end of the bm usually when a push is needed. If its a slight push (suppositories make it looser) I may not see any. Yet if I push just a little more I will drip blood every time.

Whats going on? Why would rectal meds, especially budesonide be doing absolutely nothing?

Why would I bleed only when pushing?

Waiting to see if they approve me for biologics but Ive been warned that besides bleeding for a year amongst other discomforts, my case to doctors is not severe..

For sure you dont have a hemorrhoid? Or a fissure?
Has your GI taken a look via rectoscope in office?

If there's unwillingness to address your bleeding, I would definitely consider switching providers. I had a severe flare with bleeding like you're describing and they loaded me with steroids until I could begin on a biologic (which helped significantly). Yes, the cause can be addressed, but the treatment for bleeding in one place might be the same as for bleeding found in another place. I would think the Salofalk would help with Proctitis or Fissure, so you may need to pursue something more drastic. It sounds like you're in a lot of discomfort.

I agree with quincy, your dr can do a quick scope in the office to check you out. I can't decide if you are passing hard stools or if you are constipated since you say it's at the end pushing it out.

So they did take a look about 5 months ago and found a small wound caused potentially by the tip of the enemas, so they put me on abx, cipro and flagyl which completely destroyed my gut and I had diarreah for weeks.

When looking again 2 weeks after they determined it had cleared. From then on I've been using suppositories only and I had a full colonoscopy about 2 months ago or so and no wound, no haemorrhoids and just 4cm of inflammation, rest of the colon was fine.

Nothing has changed much except Ive been on budesonide foam for about a month now but still bleed, it drips like a hemmie would yet they say there is none...

I can only assume its the UC and take the pro's word for it but I don't get it.

Im not constipated, if anything its solid but mushy consistency / bland... and if I try to make sure Im done by pushing a little at the end, boom, there's the blood.

It sounds like you need stronger meds. The antibiotics probably didn’t help matters esp. Cipro. The current meds you are on do not control your inflammation. I had similar inflammation one time and I also bled like that. Talk to doc about

Waiting to hear if I'm approved for a Biologic on the 30th..fingers crossed. I hope a biologic would help it.
