antibiotics and uc flare

Hello everyone,
Do any of you NOT go into a flare after taking antibiotics? Have sinus infection and dr gave me a z pac but I am afraid to take them, because i dont want to go on prednisone afterwards! Taking Entyvio, but I had one flare since being on it, and it was from antibiotics. Thinking about trying to do this without meds (sinus), but dont know what to do.

My flares in recent years have all followed antibiotics, unfortunately. I wish I had started on probiotics right away both times, especially Florastor (s. boulardii).

I think there's some debate about whether most sinus infections require antibiotics, but I know I've gotten very sick with them before. I've heard from folks in the Entyvio Facebook group that sometimes respiratory tract issues get out of hand, even though it doesn't suppress the immune system quite as systemically, so I'd be weighing both sides of the risk (taking antibiotics vs. not) and upping the probiotics either way.

I hope it works out well for you, one way or another.

I don't flare from antibiotics...i take zithromax and Clavulin with no issues. I always double probiotics when taking them. In Jan/Feb i was on on 2 courses of zithromax and one of Clavulin in succession for a tooth abscess...had to have the tooth pulled.

Sinus infections do not heal well on their own...and what you do not want is for it to keep getting worse.

If you are OK with mesalamine, you could ask your GI for you to use the retention enemas for a bit to keep your lower colon happy 😊

Thank you so much for your responses! The antibiotic she gave me seems pretty mild. It's a Z pack for 5 days but only 250mg. This disease is so frustrating , as you already know. Help from others who are going thru the same thing really helps alot. Also, I know everyone is different and each persons results are entiterly different too. I wish they'd find a cure. I saw they have a new medication, but not sure if it was approved by the FDA yet. I think the name was Zeposia, or something like that. dont remember if its a pill or given by infusion. Would be interesting to know what results they"ve found with that. Thanks again for your help.

You don't have to take prednisone if you don't want to! I refuse to take it any more (as does my sister with crohns). Hopefully you'll be fine. Probiotics during and after the antibiotics is a good idea.

They always give me prednisone and usually its the only thing that stops the bleeding. Does your Dr. give you something else in place of it?

I was on prednisone for 2 years while we tried different drugs and got osteoporosis as a result. I won't take it again.
Have been in remission for 3 years on xeljanz. Last time i started to flare mesalazine enemas (on top of the xeljanz) fixed it. If i flare again and enemas don't work i will switch to a different drug class (such as zeposia) . If drugs don't work I will have surgery. As my sister says, prednisone is a short term solution to a long term problem.

To add to my last post - I've never been able to use prednisone to get into remission. Only for symptom management while I wait for other drugs to work.

Thanks for letting me know about that. I also have osteoporosis and I don't want it to get worse. The Dr talked to me a little about Stelara, but I didn't like the side effects that comes with it. I have been on Entyvio for 2yrs and it was working well until recently. It's so stressful which only makes things worse. Thanks again for your help...I wish you the very best!

You...MESALAMINE ORAL AND RECTAL! And request it.
I've never nor will ever be on prednisone unless my life depended in it.

Most people don't have any side effects from Stelara.

As for taking a z-pak for a sinus infection, that was a common treatment. I say "was" because many doctors no longer treat sinus infections by initially prescribing a z-pak.

Zithromax has been over-used to the point that it's no longer very effective for many types of bacteria. What's more, the current thinking is that sinus infections often clear on their own without the need for any antibiotics, as GrittyHope alluded to.

For somebody with UC, for whom any antibiotics increase risk of flare - and nobody can predict how you will respond to any specific antibiotic - the risk/reward of taking a z-pak is even more tilted.

Try to clear the sinus infection with other means before taking an antibiotic. By this I mean steam inhaling, saline rinses or saline irrigation, decongestants, mucolytics, etc. Only if you can't clear the infection by such means should you take an antibiotic for it.

Yes antibiotics can cause flares, but it depends on which one. Antibiotics that are targeted are less of a problem. Broad spectrum antibiotics that kill many species are the worst, and should be avoided in UC patients.

Hello Everyone,
Turns out that the flare I was worried about getting from the antibiotics ended up being C-Diff. So now I am on Vancomycin. I need to take it for 14 days. I don't know how I got this, unless it's a flare that turned into C-Diff. I guess I must have picked it up somewhere, but I hardly ever go anywhere because I'm so pananoid about the Covid.
Anyway, sinus infection getting better. I have been taking alot of probiotics and drinking Kefir and I have to say I think it really helped. So, just wanted to share that with you. Taking these antibiotics is a nightmare, because it's four times a day, and because I've been so sick lately my days are very short.
