Going on Safari to S America Kruger Park in August/September. Classified as High Risk for malaria. Have been on Entyvio since March. Malaria Pills are recommended by the CDC. There are 2 brands I am researching (Malarone and Doxycycline. Has anyone taken either of these and if so any issues. I can't take the Yellow Fever Vaccine since it is a live virus. Also any suggestions on other medications to take with me? Thanks

Odd that you can't take a live virus vaccine... I was not given that warning when I took Entyvio.

Anyway... about malaria pills. When I lived in Asia for 2 years, I took the chloroquine variety when I traveled to endemic regions and it did not affect my UC, but I got some of the other side effects like altered mental status and insane dreams. It was not exactly pleasant, but it was that or risk malaria. I tolerated it. However, I was taking hydroxychloroquine which is an older generation one. Maybe malarone is better.

I personally would not take doxycycline for months because it is a broad spectrum antibiotic and will wipe out your beneficial gut flora. I took it for 3 weeks once due to a stubborn sinus infection and I flared by the second week. I can't speak for others though. UC is worsened by dysbiosis and antibiotics do that, especially the broad spectrum ones *or* taking antibiotics long-term. As if that isn't bad enough, you'd be in a foreign country where food poisoning is a possibility and you'll want your gut flora in tact to protect you.

Just my two cents.

Neither drug is great but malaria is a total nightmare. In 2008 I got malaria when I visited Nepal. I was not taking prophylaxis because it was the low season. I was one of three people who got it in Pokhara and the surrounding region!!! My initial recovery took 3 weeks but I had dormant malaria organisms in my liver for 6 months that took forever to get rid of.

Thanks for your response. I agree it is a catch 22 with the antibiotic. Just going in the right direction on Entyvio and hate to mess things up. Going late August and early September which is the end of winter season-considering taking a chance on the weather helping and skipping pills. Tough call.

I've been on malarone and was fine (no side effects at all) but that was before my UC diagnosis. I'd be wary of taking an antibiotic. You are correct that live vaccines are contraindicated on biologics including entyvio. Does south africa have yellow fever?

Doxycycline was pure hell for me. I think I lasted 3 days on it? It's such a strong antibiotic that it's used to treat potential ANTHRAX exposure. I was given it this summer because I had a bullseye rash from a tick bite. I felt like absolute crap on it.

Sara-Thanks for the info. Think I will take my chances and wear long sleeves and long pants with lots of DEET spray.

Why don't you find a safer place to vacation? (Considering that UC management has been a big issue for you lately, and you're still relatively new to Entyvio.) I think we seniors especially need to be very pragmatic about what potential hazards we can easily avoid and choose reasonable alternatives rather than take on high risk activities. Mosquitos and malaria are no joke even to otherwise healthy people. I write this knowing about a healthy young male who got malaria while on Peace Corps assignment in West Africa. First matters first, it's also better to travel where you don't need to take preventive med that could revv up UC again. Definitely take care, as if your body is a temple (as longtime dentist of mine used to say). / Old Hat (40+ yrs with left-sided UC; in remission taking Colazal)

Old Hat - I have not been to south africa but to Namibia which borders SA to the north (and does not require malaria protection if you stick to certain regions, nor yellow fever vaccine), I can honestly say it was the best thing I have ever done in my life. So I can understand the OP wanting to travel in that area. But I would personally take malarone, and not rely on DEET.

Has anyone used Malarone and if so what was your experience. Looks like I will need to take a total of 21 tablets.

I have used malarone and had zero side effects. I just took the tablets as directed. I don’t think it's associated with ibd.

No problem. It wasn't the GI issues that were the worst for me on doxy, but I did have diarrhea and some nausea. I just overall felt really awful, like I felt like I was poisoning myself. You might do better on it. Or maybe the malarone will be better. I'm someone who usually has no issues with most antibiotics.
