Hi, yes I had just started inflectra, I have had just one dose. Second dose was to have been last Wednesday. Now it is pushed to this Tuesday.
I am definitely several steps back already. I’ve started the enemas, slow progress.

D. Brasco, rather than hijack Rusty Barr's thread, I decided to bring your thread back to life. You were to see your gi, how did the appointment go? Let us know when you feel like it.

I figured Rusty didn't mind :D
well I saw the GI yesterday and we both sort of agreed that I am flunking inflectra but we don’t want to give up yet. In four weeks I will get the 10 mg/kg and see how I do on that. If I don’t get any improvement in a few days, he suggested to start prednisone to get things under control.
I'm doing one hydrocortisone enema every night. and mesalamine supps during the day (just re-started this last one)
let's see how I do. I'm trying to manage my job at the same time which is pretty stressful

What techniques do you guys use to avoid straining? At this stage i am right now feels impossible to avoid that final push.
I have tried the squatting, deep breathing, the stepstool under the feet.
I feel like straining, set me back a couple steps

Have you ever given dicyclomine (bentyl) a try? It can help with tenesmus — which I’m assuming you mean by straining? To me it feels like a muscle spasm in my sphincter/rectum. Very uncomfortable! You might ask your dr and see if he thinks it would help.

How long does it take for Pred 40 mg to make significant dent to a bad flare? From your experience

Depends. Used to see improvement in a few days. Lately, it took more like a week, if I'm remembering correctly. Feeling any better yet?
