Is it possible that Infliximab (Remicade) has stopped working although I have a good drug level and no antibodies?
I had a very good year with the drug until October when flare symptoms appeared. An increased dose was suggested and I went from 5 mg/kg to 10 at the last infusion. The day before the increase a blood test showed no antibodies and a drug level of 13 (6 to 10) is ideal. Symptoms are a bit better the last couple of days, but that is meaningless because of Prednisone.
Now the drug level must be even higher than at the low dose. Too high? More side effects? In the past I had low levels of antibodies which were not reported this time. To match symptoms my Calprotectin has been rising to the top of the intermediate level since that historic one-time happy event in January, 2022, when it was low normal.

A level of 13 isn't all that high. My level was well over 20 the last time I was tested. I asked the dr if that was a concern and he told me it wasn't a problem and that was the level my body needed. He also said I wouldn't develop antibodies with levels that high. I did a lot of searching and couldn't find any info about high levels pro or con so dunno. Just happy to be in remission smile

CCinPA I envy you for having Remicade adjusted because of mild symptoms while it was still helping you. My increased dose is in response to flare-like symptoms that can't be ignored. So I don't know if I am still in remission. Time will tell. But thank you for the reassurance that high blood drug levels are not critical, and might even be helpful.
