What should I do?

I first want to start with background info.  My first run in with GERD happened when I was 18.  I had a very slight feeling of something stuck in my throat (globus).  I ignored it and when away.  It returned two years later during my sophmore year of college.  I would have heartburn occasionally, but not on a consistent basis.  Over the next couple years I would have the globus sensation on and off w/ increased heartburn.  I'm sure it was caused by excessive drinking/smoking that I did in College.  At the end of my college career it began bothering me allot, I did some research found out what was causing it and started taking acid blockers.  This seemed to work pretty well for the next couple years, but the globus sensation would still come and go.  I'm now 26 and take 40mg of omeperazole everday, and this keeps the acid down for the most part.  But the globus sensation is very persistent and only occansionally seems to get better, but every time it does it seems to come right back.  I burp allot and have what I think is slight gastritis sometimes.  I still smoke (I know stupid), but I have cut it down to only a couple a day.  I drink on occasion, but I've been cutting that back as well.  I also have a gurling/clicking noise sometimes from the throat, and seem to get a stiff neck when its bothering me allot.  I avoid foods that give me heartburn mostly, but I still seem to burp allot no matter what I eat.  I sure it would be heartburn if I wasn't talking the acid blockers.  The thing that bothers me the most is the Globus.  The constant burping and the heartburn on occasion I can deal with, but the Globus drives me nuts.  I know I should go the doctor, but my current Job does not have health insurance.  So my question is, is there anything a doctor could do or perscribe that would help?  I'm already taking 40mg of over the counter omeprazole everday?  I know I should eat better, lose wieght(I'm about 20 pounds over wieght), stop smoking/drinking completly. And I am working on them, but its harder said than done.  Also, I know when I worry about it, the globus get worse, but i'm sure its not the hysteria kind.  Sorry this is so long.  I'm just not sure what to do anymore.

Kevin, You can get help with medicaid.Please go get an endoscope.I have choked several times.Ignored the chokes til I almost didn't make it. Went 2 c the GI.He gave me an endoscope and seen my esophogus need dilation.While down there,he took tissue samples^ have severe Gerd and had H Pylori.I am now headed to surgery because of 3strictures and my valve not working (LES)
My advice,please go.My dad waited until he had esophagus cancer and died at 50.

The thing is I make to much money to qualify for medicaid, and I doubt I would be able to pay for all the tests they will want to do.  Between the rent, car payment, student loans, and basic living expenses there isn't a whole lot left over.  I've never choked on any food, I don't have any problems swallowing food. In fact it seems to relieve the globus sensation.  I never get nauseas either.  The heartburn seems to be fairly managable with meds, although i do get mild flair ups here and there.  If it wasn't for the globus sensation I would be able to deal with all this pretty well.  Its just that annoying sensation in my throat bugs me constantly.  I know my dad has heartburn problems to.  He got a endoscope a few years ago, and they said he had acid reflux.  They put him on nexium, but he stopped taking it a few months later.  He seems to be better now, I know he still gets heartburn here and there, but I don't think its a constant thing.  I'm pretty sure I have it worse than he does, and at a younger age.  I should put myself on a strict diet and lose a little weight.  Since my symptoms aren't real bad I'm pretty sure this would do the trick, Its just easier said than done.  I love beer and all sorts of food.  It will be hard to deny myself some of things I enjoy most in life.
