GERD/LPR or PND/Stress?

I've been struggling with a burning throat/anxiety and excess mucus for a long time now. I've tried 5 types of PPI and 2 H2 Blockers as well as Gaviscon Advance, lifestyle/diet changes, Alkaline Water, Kouffman's Reflux Diet and more besides. Today I completely stopped all meds and while I feel a little worse, I also don't feel too different..! From keeping records over the last 3 months, I've not seen a pattern in what I eat or do being the cause; in fact
sometimes I did the exact same thing for 2 days in a row and would burn one day but not the next! Even lettuce and water makes me burn.

I was wondering if the burning in my throat could be caused by something else, maybe stress or the PND itself? I ask because on an evening last week, I was given a pizza and ice cream by a friend, and couldn't exactly turn it down as I had nothing else to eat. Despite these being A-List reflux causes, I didn't experience any symptoms until I drank some water 2 hours later!

Any help you can offer would be much appreciated!

EDIT: Any idea how long the H2 Blocker Cimetidine takes to work once taken with water? Silly question, but I honestly can't find any answers online so far. XD

That's so interesting that you can eat pizza and ice cream without any problems but the water will cause problems.  I am the exact same way.  I never even heard that you're not supposed to eat ice cream until I came here.  My GI told me not to eat pizza but never mentioned ice cream.  Neither one seems to bother me but water will cause burning.  I feel a little better knowing that I'm not the only one.  I find that if I don't drink water with my meals, I feel a lot better but it does cause me to get constipated.  So I guess no good deed goes unpunished.
 
H2 blockers never worked for me so I can't answer that question.  I am apparently stuck with taking PPI's.

My throat didnt start burning when i drank water until i was on the acid blockers.  They made things so much worse.  I mix a little coconut water with my plain water and it helps with the burn

I have Colon Cancer and after my surgery I seem to have less problems with my GERD although I continue to take Prilosec. 
 
Two nights in a row now I ate popcorn while watching TV and both night I paid dearly in the middle of the night with reflux and severe pain............I guess I jumped to assuming in thinking my GERD was not a problem anymore so back to watching what I eat.
 
I suspect the pizza party I attended at my daughters at dinner time did not help any.
 
When I have the reflux I start taking chewable tums and also kicked in a antiemetic.
 
Good luck and go back to step one with watching your diet.  JMHO

Daxter regarding your EDIT question about Cimetidine. I just picked this up from a medication application. Don't know if its helpful but,-- here it is:
Peak concentration 45-90 minutes.
Elimination half-life 2 hours.
Take 30 minutes before meals.
Should not take with antacids as they interfere with absorption.
Should not smoke while taking.

Thank you; if I understand right, does this mean Cimetidine works best between 45-90 minutes of taking the medicine, and takes full effect for 2 hours before it is absorbed. Sound about right?
