Actually everyone has a HH to some extent. The body has this small natural hernia to allow for the esophagus to enter the chest cavity. Those people who never get reflux just don't know they have it. Some people have larger HH than others and larger HH can add to the reflux problem, but it's not usually the only issue.
Of course those with a huge HH and their stomach and maybe intestines in their chest have an unusually large HH.
