Need help with diagnosing what's wrong with me?

Hi all, I'm going to quickly explain my symptoms and test's I've had. Hopefully someone out there can help me.

I get an upper abdominal pain after eating, literally after everything. The pain is about an inch away from my belly button (I'm not overweight at all). I can't eat lots of food anymore because of the pain.

I've had an endoscopy and It showed I have a Lax Cardia Sphincter. So I got told okay I have reflux. But I don't get heart burn in my chest at all. It's only in my upper stomach. If I push down on the area I can feel sore pain.

I've been tested for H.Pylori and I had an intolerance test that showed I'm gluten and dairy intolerant. I avoid these foods now but I still get the pain.

I'm currently taking 80mg of Omeprazole a day but still get pain after eating. My doctor has put it up because she wants to try a high amount of PPI's before considering me for further treatment (potentially operation).
The PPi's have helped a bit but again, I still get the pain.

I've tried a lot of GERD diets and basically just eat rice, chicken and vegetables.

My doctor doesn't seem to be much help to be honest. I could do with some advice. Could someone help me out please, Does this sound like typical GERD? If so, why aren't the PPI's working?

How sure are you that you have GERD?

Firstly, 80mg omeprazole is ridiculous dosage. I am surprised you can tolerate such a high dose, at 60mg omeprazole i could not breath properly. I Would switch to nexium or another ppi if you can tolerate them. If it was gerd you should see improvements if the ppi is working. I would try another ppi, but do take into consideration they take between 1-2mweeks to start working.

I would push for a PH manometry test and this will confirm if you are having reflux, unfortunately you are going through the hoops because of cost saving or nhs procedure doctors must confirm to. Really if you do the PH test this will answer your question of whether you have reflux or not.

then you can make more informed decision.

Thanks for the reply, the 80mg is only for 2 weeks, she started me on a course of 40mg a day and said if it doesn't get better in 2 weeks go up to 80mg (It's divided twice a day). I was 30mg of Lansoprazole for a few months before this. I do get a weird feeling in my throat on 80mg though!

Like I said, I have seen a bit of improvement but not as much as I expected.
I'll ask about the PH manometry test on my next appointment. Is it common to get just upper stomach pain and no heartburn with GERD?

The position you are taking about is the lower oesophageal sphincter, this is aprox midway between your belly button and your sternum.

It could be the leaking of reflux after eating that is causing the pain. As it is only connected with your eating I would presume that it is a reflux problem.

Have you ever tried when you are experiencing this, pressing in on the painful area to see if it helps?

I also agree with James, you do need to get a manometry and a PH study done, this will show if you are having reflux and if you are a good candidate for surgery.

There are many people who have regular reflux and don't really feel it burn, and with others like myself just the slightest amount will burn, we are all different. It could also be that you are having some non acidic reflux which of course the ppi's will not help, this will also show if you get an impedance PH test.

Try some gaviscon advance after all meals and see if that eases things.

Thanks for the replies, Yeah I only seem to get pain on my Lower Esophageal area, I've just tried gaviscon advanced, I seem to feel a pressure when I take it and my stomach starts pulsating. Do you have any idea why?

I assume if my doctor refers me to the GI specialist that they'll do all appropriate tests before considering me for surgery.
