Just wanted to throw this term out there.  Some of the people on this board seem to be really helpless as ot the treatment of their (alleged) GERD.  While you can certainly suffer from GERD, in ADDITION to that, you might want to have someone check you for celiac disease.  The symptoms resemble bad GERD very much, my ex-girlfriend is suffering from this.  However, since few doctors know what to look for, it's often not diagnosed for a long time.

I was really having troubles last fall and my gastro doctor told me take more Nexium. I went back to my regular doctor, who is much better and doesn't like just prescribing pills if there is another solution. She suggested several possiblities. One of them was Celiac. She included it in a blood test I had.

Turned out not be it, but it didn't take much time or effort to rule it out.

I'm thinking alot of us have systems that just can't handle alot of gluten. And I'm thinking the test doesn't always show positive, but you can still benefit from cutting back on carbs. I know someone who tested negative, but her doctor proceded as if she did have a gluten sensitivity...........and she improved a ton.
So I'm thinking everyone who has symptoms of GERD and IBS, allergies, etc., should try a low gluten or gluten-free diet.
I think alot of people just accept that if a certain test is negative, they think that's not a problem. But I think that's not true. What could it hurt to try a low or gluten free diet? That's what I would suggest.
(But......I'm having trouble practicing what I preach!!! ;))
