URGENT! Not getting enough calories!

I know I've hinted at this question before, but I can't stress enough how important this info is to me. How do I stick to a GERD diet and still get the 2000 calories a day to maintain my weight? I'm so terribly underweight that this literally is a question of my continued survival!

Please any help would be greatly appreciated.

Just a few foods to consider:
organic peanut butter on Quaker lightly salted rice cakes
hummus and baba ghanous (spelling?) on rice cakes
broiled/baked fish sprinkled with seasoned bread crumbs
goat yoghurt and milk (easier to digest than cow's milk)
smoothies (made in blender with 1/2 avocado, a banana, 1/2 c yoghurt, some
honey, a few ground nuts)
chicken and more chicken
baked yams and sweet potatoes

For a short time I used Boost Plus and added chocolate syrup with it. yeah

Ya, I just started the Boost High Protein vanilla drinks... and yet I'm still getting crazy heartburn that makes me want to die. It doesn't seem to matter what I eat or don't eat, or what meds I take. I just want to die. They had to shoot me up with morphine in the ER because the pain was so bad that it burned right through the lidocane GI cocktail they gave me... this is truly hell.
