I take OTC heartburn drugs and Tums along with the Nexium.  I found the omeprezole to work just as well as the Nexium.  Try generic Tagamet or one of the others to help the prescription drugs work.  Anxiety over my husband's health makes my problem flare up every time.  APG

I would burp and the pain would go, 15 minutes later pain again and still got those spasms in my chest not really sure if there spasms or heart palps? today Im not going to take nexium see what happens. Thanks for the advice APG.

eyes  Sure don't want to be the bearer of bad news, but you should see your Doc about Barrett's Esophagus. If you have had Reflux for years, you may have bumped up to the next level, Barrett's. Not the best news, but still managable.
I get so angry at Docs when they don't inform their Heartburn, Reflux, and Hiatal Hernia patients, that left untreated (or as is the case for many, they just don't know that what they have can get worse. If the Docs told Reflux patients that managing their condition with diet and positional pracrices, such as raising the head of their bed 6 inches higher than the foot of the bed, and Not to lie down for at least 2-3 hours after a meal, to monitor closely foods that create too much acid, and restrict them, that if they do not do these things their condition will worsen. That where they are now, is just a few inconviences, but these inconvient habits are better than chemo therspy! Yes, chemo therapy, IF reflux worsens it can become Barrett's, and Barrett's is a pre-cancerous condition.
Barrett's can cause a feeling of not being able to swallow easily.
However, not being able to catch your breathe could be a hiatal hernia. That is when part of your stomach protrudes into the chest cavity, which can then produce asthma-like symptoms, can also cause reflux (pusing the stomach contents up into your esophagus). If yuour chest feels tight, and you have had your heart tested, then you may have a hiatal hernia.
See a Gastrointerologist, tell him you have had heartburn, now you have ttrouble catching your breathe sometimes, and (if it is following a meal, tell him that too) your chest feels tight. I'm not trying to put words into your mouth, but these days you have to be your own advocate in the medical field, 'cause doctors don't always have time to diagnose us from zero, the more information you can give them, the faster you will get help.
I'm sorry if I seem pushy. I lost my Father to Esophagal cancer, and now my husband has gastritis. I worry that his will move to the next level.
Best of luck, hope you find very good help.
