Went to see my doctor about getting off sleep meds; prescribes me bupropion and trazodone

Long post ahead but I'm lost and don't know what to do.

I've been dealing with chronic insomnia for decades. As I get older (just turned 58), the insomnia has really become unbearable. On a good night, I get maybe four or five hours of sleep and I can function pretty well throughout the day. But there are many bad nights when I get little to NO sleep at all. The days following those nights are unbearable.

So about six months ago, I convinced my PCM to let me try some sleeping pills. I was prescribed with Lunexa - 3mg, daily. But as is well known, the Lunexa worked fine initially but as my tolerance to the drug built up my insomnia returned. Lately, there have been nights when I've taken 3, 4 or even 6mg of Lunexa and it had no effect on me.

The lack of sleep has made me a very unhappy person to be around. I snap at the slightest things. One recent event: I took a trip to New Orleans to visit friends in a cancer support group I belong to. When I got to my destination, I went to the car rental counter where I had reserved a car. When I got to the counter, there was no car. Although the guy behind the counter tried to help, I went ballistic. I was screaming and yelling, I was making a scene, etc. My wife was scared, she thought I was going to get arrested. I've always had a 'bad temper' but this was a nightmare. Reflecting on it now, I can't believe I was like that.

So almost two weeks ago, I stopped the taking the Lunexa and I went back to see my doctor looking for help. The Lunesta was no longer working and my insomnia was taking a toll not only on me, but those around me.

After filling out questionnaires asking me my medical history and talking with the doc for a bit, the doctor felt I had the classic signs of mild depression and generalized anxiety disorder. Accordingly, I've been prescribed bupropion, 150 mg each morning, along with 100mg of trazodone, which I am to take each night to help me sleep.

In addition to the trazodone at bedtime, I am also taking 1.5mg of melatonin ('Midnite' sleep aid, also contains lemon balm, chamomile and lavender) as well as some CBD tincture that I picked up at my local dispensary (one eye-dropper full under the tongue at bedtime).

You would think with the trazodone, melatonin, and CBD tincture that I should easily fall asleep when i head to bed at 10:00 p.m. each night. But, nope, I rarely even feel drowsy. I cannot understand how nothing seems to help.

I've been using an app on my phone to track my sleep. Here's what I've been dealing with for the past week:

Nov 9 - 3 hours of sleep
Nov 10 - 4 hours of sleep
Nov 11 - 4.5 hours of sleep
Nov 12 - 40 minutes of sleep
Nov 13 - 4.5 hours of sleep
Nov 14 - 2 hours of sleep

I am unable to concentrate or focus at work. I'm a software engineer / computer programmer by trade so this is not good. Additionally, I have to drive to the office two days a week, 1.5 hours each way. Today while driving to work, I felt uncomfortable and unsafe. I'm worried that I'm going to get into an accident and hurt myself or others.

I don't know what to do. I know these drugs can take several weeks in order to start working but shouldn't I feel at least something by now?. I'm approaching two weeks on these drugs and so far I am not feeling any therapeutic effects. None. I don't know how much longer I can keep this up.

You may need more Trazodone. I take 300 mg every night, 60 yr old female weighing around 170 lbs -- no caffeine and very little sugar in my diet. Same dosage for over 10 years, can't sleep without it - but it's definitely helped my insomnia of 40 years!

you are not sleeping despite the meds because your dr was right about your mild depression. I went tru exactly same senerio and even ambien wasn't putting me out for the entire nite. once your depression lifts-normal sleep will return. trazadone is hit or miss for many. didn't work at all for me. what did was doxepin. its in the sAME class as trazadone but really put me out. a real heavy weight is remerion. will knock a horse out but you will have trouble functioning next day. now that I am depression and anxiety free -o sleep like a baby every night -all night. I suffered tru insomnia for months and know what you are going tru. . getting drs to prescribe real sleep meds is hard these days and trazadone is their go to drug . doxepin was MUCH better for me you may want to ask about it if no success with traz. its an old antidepressant and when used in low doses is effective for insomnia. traz is the same kind of drug.

@super20dan - Can I ask what resolved your anxiety and depression? Was it the drugs, therapy, both, or something else?

I'm only asking because this bupropion I'm taking is not doing a single thing for me. Monday will make two weeks since I've been taking 150mg daily.

Monday is also my appointment with my doc and I was planning on bringing my remaining bupropion and trazodone for proper disposal. I am hoping they can prescribe something else to replace the bupropion and I'll ask about the doxepin for insomnia. I was prescribed doxepin for a while about a year ago and it did work well for a while. But because the anxiety / depression was not being treated, I guess the doxepin became less effective over time and I stopped taking it after six months.

But if I can get rid of this depression / anxiety, I'm hoping that would mean I could get off of any drugs.

The bupropion can take six to eight weeks to reach full potential. My husband takes melatonin but takes it at 6:00 pm. He takes 10mg and it works. The cbd can take awhile to start working too. Give it all a chance. What mg of melatonin are you taking and at what time?

It can all be so complicated. Do you ever meditate? I meditate in bed before I fall asleep. And it works.

PS I can't tolerate high doses of bupropion as it gives me insomnia. I think that was with 300mg.

@getting by - The melatonin I take comes from a product called 'Midnite'. It has 1.5mg of melatonin. It also contains a blend of lemon balm, chamomile and lavender. I've been taking it at bedtime.

Last night was another terrible night. I got two hours of sleep beginning at 4:30 a.m. I'm also experiencing fainting spells now. Just getting up from the couch after a cup of decaf and the news this morning, things got all 'starry', my knees buckled and down I went. My wife was able to catch me before I hit the floor.

With this extreme fatigue, inability to concentrate / focus, blurred vision, and now fainting spells, I don't think I should drive anymore. Not until I am able to get some sleep.

There's no way I can give these drugs another four to six weeks to start 'working', whatever the hell that is. My doctor better come up with another game plan when I see her on Monday or we're done.

Hi John, Have you tried ashwaganda?I used to have terrible chronic insomnia. It'll literally make someone nuts... Wasn't long after I quit taking all prescribed drugs and more normal sleep patterns returned. Now I take a smaller dose of ashwaganda at night if I feel anxious or not tired. It works. Also use Traditional Medicinals brand of Nighty-Night tea sometimes. Have to let it soak in a covered-mug for 20 minutes but it works surprisingly well. The drugs they gave me for depression, pain, sleep & anxiety just made me feel worse. They made me feel confused, daytime groggy, more depressed,cranky, dumb, heavy-feeling legs. It's like torture trying to function & work when your brain's 1/2 asleep & your body feels like collapsing. Sometimes, I go to bed early & just lay there reading after taking the herbal remedies and that helps. Hope you start sleeping well soon.

1.5 mg of Melatonin is very little. I'm a 105lb female and I have to take 6 to 10mgs (depending on other factors like my pain level/other meds ive taken)for it to have the desired effect of actually helping to put me in a sleep that lasts at least three or four hours. 3 mgs makes me slightly drowsy/I might fall into a light sleep for an hour two.

My advice is to try increasing the melatonin every few nights until you find your optimal dose for now. And then when your other medication reaches it's full potential, reevaluate and take less if you're able.

UPDATE:

Something strange happened last night. I SLEPT FOR 6.5 HOURS!

Last night, I had to do some remote work for my job and did not get to bed until 11:00 p.m. In addition to that, I took 5mg of melatonin when I went to bed. I was asleep by 11:30.

I don't know if it was the lack of sleep that finally pushed me over the edge, whether it was the later bedtime, or if it was the 5mg of melatonin. Or maybe it was all of the above?

I will try the same again tonight:
(1) 100mg of trazodone at 10:00 p.m.
(2) Bedtime at 11:00 p.m.
(3) 5mg of melatonin at bedtime.
(4) Out of bed at 6:00 a.m.

I've been on Trazadone for 18 years now and it sounds like you need a higher dose. My Dr. works with me when I think I need to go up on it. It helps me sleep through the night and helps with my light anxiety. Good luck John

I highly recommend you try Remeron. It will work on the first night and should knock you out for 9 or more hours. Trazadone kept me up the one time I tried it and made me feel really weird; it was not a pleasant experience. Melatonin does nothing for me (it seemed to keep me awake, if anything). And Wellbutrin can cause insomnia, in addition to really crappy sleep filled with strange dreams and nightmares.
35 yrs old; dxd UC March 2007 (couldn't get scope all the way thru). 9-29-16: chronic & active proctosigmoiditis (infectious cause). Reoccuring campylobacter & c diff. Oct-Dec 2016. Remission 12/25/16 until started smoking again in 2018 after 9 yrs quit. Maintenance: Delzicol, 6, 2xday; Rowasa nightly. Remicade 7/25/19 (worked 2 months). 50 mg pred 10/29/19. Began Humira & Imuran (150 mg) 11-16-19

UPDATE:

My joy of sleeping was short lived. I got only two hours of sleep last night, beginning at 2:30 a.m. Woke up at 4:30 a.m. and that was it; there was no going back to sleep.

Had my appointment today with the therapist. She read through my CBT-i sleep logs and was quick to point out that I'm having a very difficult time. She must be brilliant.

All she did was then give me a couple of handouts regarding calming / relaxing the mind and ways to distract the mind with useless tasks, like counting ceiling tiles and the like.

I brought notes that she was going to pass along to my PCM:
(1) I've requested that we try Remeron as a sleep aid versus the Trazodone.
(2) I've requested that we look at alternatives to Bupropion. I've read that if one has anxiety / stress issues, then Bupropion is not really a good antidepressant to prescribe.
(3) I've requested a blood test to look at my thyroid levels (T3, T4, TSM). I want to rule out a physical cause of my insomnia, if possible.

So now I'm waiting to hear back from my PCM. In the meantime, I am continuing on with my current medication routine and schedule:
- 100mg of trazodone at 10:00 p.m.
- A dropper full of CBD tincture under the tongue at 10:30 p.m.
- Bedtime at 11:00 p.m., 5mg of melatonin at bedtime.
- Out of bed at 6:00 a.m.

You might try taking the trazodone earlier than 10 pm. I take trazodone as a sleep aid and my psychiatrist has recommended taking it 1.5–2 hours before bedtime.

I tried Wellbutrin and had to stop as it made me incredibly anxious. I agree that it might be contributing to your anxiety and sleep problems.

I hope they give you the Remeron and that it works for you! I know how horrible insomnia is from experience! If you do take the Remeron, they recommend you are in bed or headed to bed when you take it. It can kick in somewhat quickly. Yeah, you need something more than meditation/calming exercises right now!

Oh, and also, I only need to take half of a 15 mg Remeron for it to knock me out (so 7.5 mg). One possible side effect when you first start it is bad nightmares. That did happen to me the first time I took it many years ago but it did not happen to me when I took it earlier this year. It only lasted a couple nights the first time though. Just a heads up.

hd ryder-sorry not to answer sooner. what cured my anxiety and depression and insomnia is celexa. its a weaker form of Lexapro. it took 8 weeks to take effect but I am totally anxiety -depression free and sleep like a baby. even take naps in afternoon. someone suggested Remeron -yes its extremely effective but VERY strong. chances are you WILL sleep but be quite hung over next day. the usual dose is 15-start with 7.5 and see how it goes. you can take the other half if you awake after only 2 hrs or so. that's how I did it. someone also suggested a stronger dose of melantoian-yes ! safe to take up to 10mg at once . 6 usually worked for me. with my severe insomnia I would go 2 days with little sleep-then by 3rd day I was so exhausted I would sleep 5 or 6 . repeat cycle over and over. truly a terrible situation. keep us informed how things go

UPDATE: Hi folks - I thought I should check in and let you all know how things have transpired in the past couple of weeks.

After sticking with the Brupopion for three weeks, and averaging 2 to 3 hours of sleep a night, I had had enough and insisted on speaking to my doctor for a change in prescriptions. That was on Nov 19th. Unfortunately, when I contacted the clinic, I learned that my PCM was out on vacation for a couple of weeks, but they were kind enough to let me come in and speak with another doctor.

The new doctor was very attentive and listened to what I had to say, including my suggestions for alternative drugs and requesting a thyroid blood test. Although he completely sympathized with me and and the major insomnia problems I was having, he felt that replacing the Trazodone with Remeron was a bit premature. He felt that the Buproprion was the cause of my insomnia and he replaced that prescription with Lexapro. Within two days of stopping the Buproprion, my sleep improved tremendously. I continue to track my sleep on a CBT-i application I have on my phone and my sleep efficiency over the past few days has exceeded 80%. I am consistently getting 6 or more hours of sleep a night.

Today, I had a follow-up with that new doctor as he wanted to see how I was doing on the new meds. Since I am doing well, at least for right now, he recommended I stay on the Lexapro / Trazodone combo for the next six months at which time we'll do another follow-up and possibly look at possibly reducing these meds.

While at the clinic today, I also had some lab work done to (1) Check my thyroid levels, and (2) Check my vitamin D levels (which were considerably low during my last physical in Feb, 2019.

So that's where I am. I'm still not sure if I really have an anxiety / stress / depression problem. All that I know is that I'm getting sleep at night, and I can't ask for any more than that.

Glad to hear your sleep has improved! I was wondering how you were doing. I think insomnia can make anyone feel depressed and/or anxious.
