I'm told that the trend is to spread out biopsies to every 2 years or so. I'm in kind of the same boat but I am looking at another biopsy since my last in 2018....
Being under the care of my uro and my case pretty stable and currently following an AS program, I'm scheduled for my annual MRI in March 2021. I last saw my urologist in person in August 2020. I am...
I guess I missed this but I'll try to check it out. One of the few AS guys hanging around....
Welcome to the forum. Like some others have said, you may be in the gray area. There are so many factors that go into this. Even as one who has not has the easiest biopsies in terms of SIRS and other...
55...
As far as PSA Density goes, yes that would be a good number. As the prostate gets bigger, if the PSA stays the same, the Density will get lower. I feel I'm going through that myself....
Being on AS myself, my concern is the high involvement of the positive cores. I would also wait for Epstein and genomics if you can swing it. Whether a GS 6 can metastasize would be less on my mind....
Congratulations Booster. Similar situation for me. On AS for 3 years. Not the biggest meat eater prior, but have gone vegetarian, (no meat) for 3 years. (Grated cheese and occasional fish...
I think a Pi- Rad 4 or 5 definitely requires a biopsy....
I've been on AS for close to 3 years. Currently, PSA every 3 months enrolled in PASS Study; DRE at least once a year; MRI annually and biopsy discussion if something dramatically changes. I take it...
Good for you Toyoung. I'm only closing in on 3 years but taking it one day at a time.PSA staying low and MRI's looking good....
I think just go to your GP for PSA and DRE until something indicates that you need to see a uro....
Latest PSA result came in at 2.7. MRI came back no peripheral zone and no central gland PI-RADS 3-5 lesions. All other readings none or normal. This report is similar to last year's. No biopsy...
My latest PSA taken this week showed a slight drop to 2.7 from the 4ish I’ve been running for the past year or so. Scheduled for my MRI on 2/19/20 which will be more important....
I don't think AS with 2 cores 3+4 with high involvements....
I have been on AS almost 3 years. My data is below. My urologists at this point have never pushed surgery and I am being watched closely. Coincidentally, I'm getting my PSA drawn today and have an...
I have a superstition about getting my PSA in the same lab. It seems that's common from what I read on this board. Does anyone have the same superstition about getting a 3T mpMRI in a different...
This is interesting and gave me a headache. I remember the 33% of total cores cutoff from the AUA 2017 or 2018 Report that my urology surgeon Dr. Martin Sanda was part of. I'm also enrolled in the...
Different place seem to have different guidelines, 2,3,4? Then if they all have less than 50% involvement, that's good. Over 50% in any core then AS probably not likely. Even in my case, I had 2...
An MRI and if possible a genomics test can give additional information to determine in borderline cases whether AS is practical or not....
I had 3 positive cores on the 1st bx and 4 the on 2nd bx a year later. The 4 on the 2nd one were all on the apex side. I work with a medical urologist and a urology surgeon and it doesn't seem to...
Good luck and positive thoughts....
Thanks for these links. Yes a nationally ranked hospital is a good thing....
Hello Vic, How many cores from the 2/21/19 were positive? Also, what were the volume % of each of those positive cores? That MRI looks good....
BBQdude. I like to go 10 days just out of being extra conservative. I wouldn't panic. FWIW, I was also told that unless the MRI showed a Pi Rads 4 or higher, it wouldn't be useful to have an MRI...
I'm currently going to Emory St. Joseph's as well and am presently following AS. I'm glad to hear your case went well....
Hello Hugo, My PSA is known to bounce around a bit. Definitely keep an eye on it and keep a record....
I've been giving a urine sample every uro visit so practically at least twice a year. I also have been giving a post-DRE urine sample twice a year as part of the PASS study I'm in....
Wow that’s a dramatic improvement I’d say. Good news....
I consult with one of the authors listed, Dr. Martin Sanda, as part of my AS program. I'm also part of the PASS Research Study that Emory is involved in. I'm glad that my post-DRE urine samples are...
Welcome 2019 Guy, I would be interested in the Pi-Rad 5. Was your MRI at 3T? I'll guess it was. Other than that, I'd say AS. Of note, I've had 2 MRI's and the results were different although I had no...
JH confirming a GS 6 is good news. I'm not sure about the 70% however. Regarding the timing of the MRI, I had my MRI 14 weeks after my bx. I know that they wouldn't do a surgery in that period of...
Hello mufjem, In my case, I was able to skip a biopsy this year and than plan is to do an 3TmpMRI annually. There seems to be a slight shift away from biopsies as long as the annual MRI and quarterly...
I'm on AS at Emory University/St. Joseph's Hospital in Atlanta. My PSA is known to bounce around as well. Good to hear from you again. My second MRI turned out better than the first. Good luck....
Hello BD, I was able to get the JH opinion and OncoDX (genomic) in about a month. Any talk about an mpMRI?...
Welcome beaverduck. Is there a college family feud in your home? You don't show the % in each of your 4 cores. I think you are taking the correct steps with JH opinion, genomics and MRI. That's...
Red....
In a way, I feel lucky that my PSA jumped to 5.2 after 15-20 years of under 2. Made it easier to justify the Urolologist visit. Never thought what would be if that jump never happened. As it stands,...
I like that the number went back down a bit. Hopeful that is the range you live in. I don't want steady increases. I think the MRI and probable repeat biopsy's are more significant going forward....
Welcome Dazed and Confused. Your data is similar to mine. I agree with a second opinion to Johns Hopkins. Maybe look into an OncoDx genomics test but that can be expensive. Check with your insurance....
Dr. Hershatter is laid back and pretty good at explaining the different treatments so the meeting should be helpful....
Welcome. I have met with both Drs. Sanda and Hershatter multiple times as I continue on my AS journey at least for the moment. I was referred to Dr. Sanda by my regular urologist 2 years ago. What is...
My first abnormal PSA was 5.2. I went to the urologist at that point and he had me retest what amounted to 3 times, (two of these with additional fPSA included) over the next 12 months before...
I would seriously look for another urologist. Your experience does not seem right to me....
I think you in good shape Hope. See what the MRI shows. See what your medical team thinks. I'll read that new Dr. Scholz myself....
My uro was happy with 19. If I recall, he was be more concerned if the number was in around 40 or so. Take it as another good data point....
Great news. What was the OncoDX score out of curiosity? I was very low also. Good luck!...
From the age of about 40 to 54, my PSA ranged from 0.9 to 1.9. Always under 2. Then at 55, boom 5.2. You are doing the right things in a good place. Stay calm....
I have had two biopsies. The first one I had oral pre-medication a few days before and spiked a fever less than 12 hours after the procedure sending me to the hospital where i send a few days in...
I agree that sounded like a good visit. The MRI several month's after the first biopsy is similar to my time frame. More information for all. The confirmatory biopsy is usually about 12 months later...
