Hi I am new here and really join to get a little advice for my wife . In April her home health NP noticed lumps on her neck . The NP ordered blood work and her TSH level was 0.08 . The nurse spoke to us about Graves Disease . She then had a Ultra sound which showed a goiter on the right side and 6 nodules on the left . Then went to ENT he said thyroid needs to come out but first go to a Endo doctor . Blood work levels low again . Need biopsy on goiter . Did that benign . Then you need to see a Nuro doc . Sept first still no results of blood work . Wife is seeing double , haveing a hard time holding things . Pain , hard time swallowing . Just did more blood work 0.07. And a Nuclear Thyroid Scan and Uptake last week . Found some results on her patient portal online . Says she is Hyperthyroidism . No image results yet . She still has not been prescribed any meds for hyperactive and we are told the thyroid needs to be under controll before it can be removed . Is this true . Any guidance or thoughts would be appreciated .
Low TSH means hyperthyroidism, the cause most probably autoimmune disease called Graves disease or Hashimoto's disease, But to know that for sure you need to test for thyroid antibodies; TSI, TGab and antiTPOab.
I am not a doctor so I only share my experience, so I think that you need to make an appointment with an Endocrinologist, a doctor who specializes in Endocrinology.
Make sure to go with the ultrasound and biopsy results, based on these and the tsh, the endo will make a decision as to the steps of the treatment, since performing a thyroidictomy not always the first step, specially if not all of the gland is nodular or has tumors, many times the nodules if benign shrink by them selves only by restoring the tsh to normal values.
There are many treatments to hyper, there are the anti thyroid medications which will bring the TSH higher, but they have certain side effects, there is also the RAI(Radio active iodine) which will make you hypo to the rest of your life and thus will need to take Levothyroxine and the final option is a total or partial thyroidictomy depending on the type and location of nodules and tumors if there are such !

Thank you for the advise .Yes her Endo has agreed with her nurse and said she has Graves Disease. I guess we just wait to see what the Endo says on the 10th next month . She has been dealing with this Endo for six months and no Meds to help with symptoms and once the thyroid in back to normal the Endo's plan is full Thyroid removal by the ENT. Just seems like a long time waiting . I enjoy this site . Thanks again
