Migraine Dissertation Study (updated)

Hello,Everyone --

First of all, for those of you who have tried to contact me in the past two months and have yet to hear a response -- I'm sorry!!!  Unfortunately, not only have I had email troubles, but my computer died and I lost quite a bit of material.  In short, I'm not ignoring you and sorry for the delay!
 
The following is my original posting from earlier. . .I need only 15 or so more participants -- I would truly appreciate any help you can give me!  Thanks and I hope you all have a great holiday!
 
My name is Kimberly Kono and I am a doctoral student in Health Psychology. I am currently researching the impact of chronic pain on the lives of those who suffer from it. I am trying to recruit individuals for my dissertation study and would like to know if anyone could please help me by completing five forms?
 
I am no stranger to chronic pain. I have been suffering from migraines since I was 13 and my father has been diagnosed with degenerative joint disease-- therefore, my research is of both professional and personal interest. If you are interested in participating in my study, please contact me via email. Individuals will be given $10 for participating in my study.
