Ive got a new one - I am here at work tired as heck from the topamax which by the way is helping some but not completley, I at the begining of a migrane anticipating a full blown one. I do not have abortive meds other than vicodin and I can not take that at work so i think i am in for a big treat in an hour or so. I am sitting in an office now with the lights off. Any suggestions other than meds? Does anyone else just "know" the difference between a real BAD one and a semi bad one before it really hits. Cuz i can just tell this one is going to be BAD!

You can tell. If you think it is coming on I would try Ibuprofen,tylenol, something. Ibuprofen used to work for me if I caught it early enough... I mean just a twinge.. otherwise it was like eating a candy necklace.
Is there any way you can go home? Do you have a neurologist that you see? I have mine paged if I get one on the weekend and he will call something in to a pharmacy for me,. like an injection or something you put under the tongue or something.... I feel so bad for you right now,... I wish I could help.,..

right hip 1 inch higher than left (think me standing on the brake during accident caused this)
left shoulder lower than right
Glucosamine for pain in back (doc said try this in addition)
TENS unit for home... seems to help some when I am standing for long periods of time
Facet injections 16 (8 on each side) didn't help)
Lumbar Medial nerve branch injections (radiofrequency burning on Wed)
axert (migraine abortive)
Imitrex for migraines (injections only work)
paxil
xanex for sleep
Zonegran (nerve pain)
Celebrex
Darvocet

Sorry, this is too late to help for this time...
Try to keep some uh, what is the name Sara....the new over the counter abortive GelState Migraine...it is a liquid that you get and put under your tongue, hold there and then swallow. It doesn't taste great but is not too terrible.
It won't knock you out like some of our prescription medications and it will give you an opportunity for relief. From what I remember it doesn't cost but a few dollars and you can take a couple of doses for one migraine.
Try that next time when you are at work or in a situation where you can not afford the drowsy doped up feeling...

Can you take something like Zomig or Amerge (triptans)? Those usually will keep the migraine away long enough for me to get home. Another trick is Aleve or Tylenol (or both together) with Gravol (or Dramamine). The Aleve / Tylenol will sometimes take the edge off and Gravol / Dramamine settles my stomach so that I can digest the other two.
If you get a prescription for triptans i find that either nasal sprays or rapid melts work best for me because my stomach stops absorbing things.
My advice is to figure out what works and carry it with you. Don't get stuck out somewhere. It only took me a few times of calling my husband to drive the hour to pick me up before I started carrying my medications with me everywhere.

Thanks for the adivice. I have tried imitrex and maxalt. only oral though I get extremley sick when I get migranes so it is pretty difficult to take oral medications. I will as my doc about the nasal sprays or the sublingual tabs. The OTC med you were talking about is somthing i am definetly giving a try. I ended up going straight from work to the ER where I was until 8 pm. about the neurologist, if the topamax does not work my doc is going to give me a referral to go see one----- Thanks for all the advice!!
It sounds like you're on the right track Wenmarie.
Another trick I have found is to have a pain management plan. When the migraine starts I begin with a Zomig, then I move on to an OTC or Toradol and then finally 2 Tylenol, Ativan and 2 Toradol. I switch the OTC medications every two days to prevent rebound headaches. It doesn't always work but it's some stability and it gives me the assurance that I have a plan. It also means I know I have enough medication to stop the migraine myself without going to the ER.
I'm sorry you ended up in the ER, but if it was a whopper it probably got you the medications you needed.

If you tend to get nauseated, you might want to get some anti-nausea medication on hand to take and then also some migraine medications that you can take other ways....not oral medicaitons.
For example, migranal-is a nasal spray made with the same ingredients as DHE, imitrex, zomig also have nasal sprays...Imitrex also have injections that you can administer yourself at home and while they are a tad bit painful, I have found that they are the best option for me when the migraine episodes get ugly.
